动态心电图在老年精神分裂症患者无症状心肌缺血及心律失常诊断中的应用

Application of dynamic electrocardiogram in diagnosis of asymptomatic myocardial ischemia and arrhythmia in elderly patients with schizophrenia

目的探讨动态心电图在老年精神分裂症患者无症状心肌缺血及心律失常诊断中的应用价值.方法对182例老年精神分裂症患者进行常规心电图和动态心电图检测,比较两种检测方法对心肌缺血及心律失常的检出率,对不同类型心律失常的检出率,对心肌缺血检出部位及ST段抬高、压低的检出率.结果 动态心电图对心肌缺血及心率失常的检出率均显著高于常规心电图(P<0.01),对室性二/三联律、室性早搏成对、房性早搏二/三联律、房性早搏成对、房室性传导阻滞、心房纤颤、短阵室性心动过速的检出率均显著高于常规心电图(P<0.05或0.01),对心肌下壁及侧壁的检出率显著高于常规心电图(P<0.01),对ST段抬高、压低的检出率显著高于常规心电图(P<0.01).结论动态心电图对老年精神分裂症患者无症状心肌缺血及心律失常的诊断价值更高,可为临床医师对患者进行心功能评估及治疗方案的制定提供可靠依据,值得推广应用.

Objective To analyze the application value of dynamic electrocardiogram in diagnosis of asymptomatic myocardial ischemia and arrhythmia in elderly patients with schizophrenia.Methods 182 elderly patients with schizophrenia were tested by conventional electrocardiogram and dynamic electrocardiogram.Compare the detection rates of myocardial ischemia and arrhythmia between the two detection methods,the detection rates of different types of arrhythmias,and the detection rates of myocardial ischemia detection sites and ST segment elevation and depression.Results The detection rate of dynamic electrocardiogram on myocardial ischemia and arrhythmia is significantly higher than that of conventional electrocardiogram(P<0.01).Detection rates of ventricular two/triple rhythm,paired ventricular premature beats,atrial premature beats two/triple rhythm,paired atrial premature beats,atrioventricular block,atrial fibrillation,short-ventricular tachycardia were significantly higher than conventional ECG(P<0.05 or 0.01).Conclusions DCG is more valuable for the diagnosis of asymptomatic myocardial ischemia and arrhythmia in elderly patients with schizophrenia.It can provide a reliable basis for clinicians to evaluate patients'cardiac function and formulate treatment plans.
