摘要
目的探讨视频脑电图及振幅整合脑电图(aEEG)在低血糖足月儿中的特点,以辅助临床医师评估不同低血糖患儿脑损伤的可能性。方法选取2019年1月~2019年4月本院新生儿科收治的36例低血糖足月儿为低血糖组,按比例选取同时期入院血糖正常,但存在高危因素的新生儿作为对照组。入院后低血糖组患儿血糖稳定后行aEEG检测6h,对照组入院后72h内行aEEG检测6h。两组患儿均于行aEEG后12h内行VEEG检测。对视频脑电图及振幅整合脑电图的特点采用秩和检验,组间比较采用F检验。结果①两组新生儿母亲妊娠期血糖升高比例差异有显著性(P<0.05)。②aEEG周期性评分及总分差异有显著性(P<0.05)。③两组患儿视频脑电图中均未监测到异常脑电图,无爆发抑制、尖棘波发放或节律慢波发放。结论低血糖患儿的视频脑电图无特异性改变,振幅整合脑电图改变以周期性及总分改变为主。
Objective To explore the features of VEEG and aEEG in full-term infants with hypoglycemia in order to assist clinicians to evaluate the possibility of brain injury in infants with different hypoglycemia.Methods Thirty-six full-term infants admitted to the Neonatology Department from January 2019 to April 2019 were selected as the hypoglycemic group.The newborns with normal blood glucose level but with high risk factors were selected as control group.The infants in the hypoglycemic group were admitted to the aEEG test 6 h after their blood glucose stabilized,and the control group were performed aEEG for 6 h within 72 h after admission.Both groups were tested for VEEG within 12 hours after aEEG.The characteristics of VEEG and aEEG were examined by rank sum test,and the comparison between groups was examined by F-test.Results ①There was significant difference in the proportion of elevated blood glucose among neonatal mothers in the two groups during pregnancy(P<0.05).②There was significant difference in the periodic score and total score of aEEG(P<0.05).③The abnormal electroencephalogram like burst inhibition,sharp ratchet wave release or rhythmic slow wave release were not been found in each group.Conclusion There was no specific change in VEEG in infants with hypoglycemia.The changes of aEEG are mainly in periodicity and total score.
