摘要
目的探讨短时授精是否可以改善受精状况、胚胎质量和胚胎种植能力。方法回顾性分析2018年9月至2019年6月在唐都医院生殖医学中心行体外受精-胚胎移植(IVF-ET)助孕的患者资料共2265个周期,根据授精时间长短分为短时授精组(n=1066)与常规IVF授精组(n=1199),比较两组患者的受精率、正常受精率、优质胚胎率、囊胚形成率及胚胎种植率差异。结果与常规IVF授精组相比,短时授精组的受精率(81.79%vs.77.79%)、正常受精率(73.95%vs.68.78%)显著增加;进一步细化受精率指标显示,短时授精组的2PN受精率显著高于常规IVF授精组(P<0.01),而异常受精指标3PN受精率和0PN受精率则显著低于常规IVF授精组(P<0.01)。D3胚胎结局方面,两组之间的优胚率(55.93%vs.56.09%)无显著性差异,但是短时授精组的胚胎种植率则显著高于常规IVF授精组(39.98%vs.30.48%)(P<0.01)。囊胚培养结果显示,与常规IVF授精组相比,短时授精组的囊胚形成率(73.64%vs.61.47%)、优质囊胚率(66.55%vs.54.20%)和优质囊胚形成率(49.01%vs.33.32%)均显著升高(P<0.01)。结论短时授精可以显著改善受精率、囊胚质量和胚胎种植能力。
Objective:To explore whether short-term insemination can improve fertilization status,embryo quality and embryo implantation ability.Methods:The data of 2265 cycles of patients undergone IVF-ET assisted pregnancy at Tangdu Hospital Reproductive Medicine Center from September 2018 to June 2019 were retrospectively analyzed.The patients were divided into short-term insemination group(n=1066)and the conventional IVF insemination group(n=1199)according to the time length of fertilization.The fertilization rate,normal fertilization rate,high-quality embryo rate,blastocyst formation rate and embryo implantation rate were compared between the two groups.Results:Compared with the conventional IVF insemination group,the fertilization rate(81.79%vs.77.79%)and normal fertilization rate(73.95%vs.68.78%)in the short-term insemination group were increased significantly.Further refinement of the fertilization rate showed that the 2PN fertilization rate in the short-term insemination group was significantly higher than that of the conventional IVF insemination group(P<0.01),while the abnormal fertilization rate of 3PN or 0PN was significantly lower than that of the conventional IVF insemination group(P<0.01).In terms of Day 3 embryo outcome,there was no significant difference in the high quality embryos rate(55.93%vs.56.09%)between the two groups,but the embryo implantation rate of the short-term insemination group was significantly higher than that of the conventional IVF insemination group(39.98%vs.30.48%)(P<0.01).The blastocyst culture results showed that the blastocyst formation rate(73.64%vs.61.47%),high-quality blastocyst rate(66.55%vs.54.20%)and high-quality blastocyst formation rate(49.01%vs.33.32%)in the short-term insemination group were significantly increased compared with the conventional IVF insemination group(P<0.01).Conclusions:Short-term insemination can significantly improve the fertilization rate,blastocyst quality and embryo implantation ability.
