低频脉冲电刺激联合药物治疗周围性面神经麻痹的疗效及对面神经功能的影响 被引量:20

摘要 目的研究低频脉冲电刺激联合药物治疗周围性面神经麻痹的疗效及对面神经功能的影响。方法采用前瞻性研究方法,选取2015年6月至2019年6月芜湖市第一人民医院收治的60例周围性面神经麻痹患者,按照随机数字表法将患者分为两组:对照组和研究组,每组各30例。对照组患者单用药物治疗,研究组患者联用低频脉冲电刺激与药物治疗。比较两组患者治疗前后面神经麻痹程度评分、面神经功能评价量表评分(SFGS)、面神经功能指数(FNFI)、肌电图潜伏期及患健侧波幅比值以及治疗后总有效率。结果治疗后,研究组患者的面神经麻痹程度评分、眼裂闭合评分、口角偏斜评分、面神经功能指数、总有效率依次为(73.82±14.96)分、(27.03±6.29)分、(30.98±9.36)分、(0.93±0.12)、96.67%,对照组依次为(56.34±13.31)分、(49.01±7.32)分、(42.93±9.74)分、(0.80±0.14)、76.67%。对比对照组,研究组患者治疗后面神经麻痹程度评分、面神经功能指数更高,面神经功能评价量表评分(27.03±6.29分vs.49.01±7.32分、30.98±9.36分vs.42.93±9.74分)与口轮匝肌(2.98±0.38 ms vs.3.29±0.41 ms)、鼻肌(2.82±0.38 ms vs.3.12±0.40 ms)、眼轮匝肌(3.08±0.48 ms vs.3.69±0.47 ms)的神经肌电图潜伏期更低,而患健侧波幅比值更高(P<0.05)。结论低频脉冲电刺激联合药物治疗周围性面神经麻痹具有显著疗效,可有效减轻患者面神经麻痹程度,促使其面神经功能快速恢复,并改善肌电图。 Objective To study the efficacy of low-frequency pulsed electrical stimulation combined with drugs in the treatment of peripheral facial paralysis and its effect on facial nerve function.Methods A prospective study was conducted to select 60 patients with peripheral facial paralysis treated in Wuhu First People's Hospital from June 2015 to June 2019,and the patients were randomly divided into two groups:the control group and the study group,with 30 cases in each group.Patients in the control group were treated with drugs alone,while patients in the study group were treated with low-frequency pulsed electrical stimulation and drugs.Before and after treatment,the facial nerve paresthesia score,facial nerve function evaluation scale score(SFGS),facial nerve function index(FNFI),the electromyographic latency,the amplitude ratio of the affected side to healthy side and the total effective rate were compared between the two groups.Results After treatment,the facial nerve paralysis score,eyelid closure score,mouth deviation score,facial nerve function index,and total effective rate of the study group were(73.82±14.96)points,(27.03±6.29)points,(30.98±9.36)points,(0.93±0.12)points,and 96.67%respectively,and the control group(56.34±13.31)points,(49.01±7.32)points,(42.93±9.74)points,(0.80±0.14),76.67%.Compared with the control group,the patients in the study group had higher post-treatment facial nerve paresthesia score and facial nerve function index,lower facial nerve function function evaluation scale scores(27.03±6.29 vs.49.01±7.32 points,30.98±9.36 vs.42.93±9.74 points),lower electromyographic latency of orbicularis muscle(2.98±0.38 ms vs.3.29±0.41 ms),nasal muscles(2.82±0.38 ms vs.3.12±0.40 ms)and orbicularis oris muscle(3.08±0.48 ms vs.3.69±0.47 ms),and higher amplitude ratio of the affected side to healthy side(P<0.05).Conclusion Low-frequency pulsed electrical stimulation combined with drugs has a significant effect on peripheral facial paralysis,which can effectively reduce the degree of facial paralysis,promote rapid recovery of facial nerve function,and improve electromyography.
