小切口微创手术治疗跟骨关节内骨折的疗效探讨

Discussion on the Therapeutic Effect of Minimally Invasive Surgery on Calcaneal Intra-articular Fractures

目的探究小切口微创手术治疗跟骨关节内骨折的疗效。方法方便选取2017年1月—2018年12月该院收治的跟骨关节内骨折患80例,依据手术方式不同分为研究组和对照组,每组40例,对照组进行外侧L型切口的常规手术治疗,研究组则进行小切口微创手术治疗。观察对比两组患者的手术情况、疼痛程度以及并发症的发生情况。结果研究组的手术时间(63.8±6.2)min、术中出血量(72.4±8.7)mL以及骨折愈合时间(11.5±1.4)周均短于对照组(94.2±8.8)min,(93.5±12.7)mL、(13.3±1.5)周(t=18.269、15.918、9.127,P<0.05);研究组的AOFAS评分(86.7±12.4)分高于对照组(73.5±10.3)分(t=15.915,P<0.05),且VAS评分(3.1±0.3)分低于对照组(5.3±1.4)分(t=10.183,P<0.05);研究组的并发症发生率(2.5%)少于对照组(20.0%)(χ^2=4.935,P<0.05)。结论将小切口微创手术应用于跟骨关节内骨折的临床治疗中,治疗效果较为理想,缓解了患者的疼痛程度,促进了患者踝关节的功能,降低了患者的并发症发生率。

Objective To explore the effect of minimally invasive surgery on the treatment of intra-articular calcaneal fractures.Methods Eighty patients with intra-articular calcaneal fractures treated in our hospital from January 2017 to December 2018 were selected and divided into a research group and a control group according to different surgical methods,with 40 cases in each group.The control group performed a lateral L-shaped incision conventional surgical treatment;the study group was treated with minimally invasive surgery.Observe and compare the operation,pain and complications of the two groups of patients.Results The operation time of the study group(63.8±6.2)min,intraoperative blood loss(72.4±8.7)mL and fracture healing time(11.5±1.4)weeks were shorter than the control group(94.2±8.8)min,(93.5±12.7)mL,(13.3±1.5)weeks(t=18.269,15.918,9.127,P<0.05);the AOFAS score of the study group(86.7±12.4)points was higher than that of the control group(73.5±10.3)points(t=15.915,P<0.05),and the VAS score(3.1±0.3)points was lower than the control group(5.3±1.4)points(t=10.183,P<0.05);the incidence of complications(2.5%)in the study group was less than the control group(20.0%)(χ^2=4.935,P<0.05).Conclusion The application of minimally invasive minimally invasive surgery in the clinical treatment of intra-articular calcaneal fractures is ideal.It relieves the patient’s pain,promotes the patient’s ankle function,and reduces the patient’s complication rate.
