血管内介入栓塞术治疗隐匿性脑血管畸形出血的疗效及安全性

Efficacy and safety of endovascular embolization for occult intracranial vascular malformations hemorrhage

目的探讨血管内介入栓塞术治疗隐匿性脑血管畸形(OVMS)出血的疗效及安全性。方法采用回顾性研究方法,选取2015年10月至2019年6月广元市第一人民医院收治的134例OVMS出血患者,根据手术方法不同将其分为对照组与试验组,每组各67例。对照组患者采用手术切除结合颅内血肿清除术治疗,试验组患者采用血管内介入栓塞术治疗。比较两组患者的临床疗效、生活功能分级标准(ADL)以及并发症情况。结果试验组患者的治疗总有效率(92.54%)明显高于对照组(73.13%),差异具有统计学意义(P<0.05);试验组患者的总并发症发生率(4.48%)明显低于对照组(26.87%),差异具有统计学意义(P<0.05);试验组患者的生活功能ADL分级中,Ⅰ、Ⅱ级占比分别为58.21%和31.34%,均高于对照组的29.85%和16.42%,差异有统计学意义(P<0.05)。结论血管内介入栓塞术治疗OVMS出血,临床效果显著,能够促进患者生活功能快速恢复,减少并发症,安全有效,可借鉴应用。

Objective To investigate the efficacy and safety of endovascular interventional embolization for occult intracranial vascular malformations(OVMS) hemorrhage.Methods A retrospective study was conducted to select 134 patients with OVMS hemorrhage admitted to the First People’s Hospital of Guangyuan City from October 2015 to June 2019.They were divided into the control group and the experimental group according to different surgical methods,with 67 cases in each group.Patients in the control group were treated with surgical resection combined with intracranial hematoma removal,and patients in the experimental group were treated with endovascular embolization.The clinical efficacy,activity of daily living(ADL) scale,and complications between the two groups of patients were compared.Results The total effective rate in patients in the experimental group was 92.54%,which was higher than 73.13%(49/67) in the control group,the difference was statistically significant(P< 0.05).The total complication rate of patients in the experimental group it was 4.48%,which was lower than 26.87% in the control group,and the difference was statistically significant(P< 0.05).In the ADL scale of the experimental group,the grades Ⅰ and Ⅱ were counted for 58.21% and 31.34% respectively,which were higher than the control group’s 29.85% and 16.42%,the differences were statistically significant(P< 0.05).Conclusion Endovascular interventional embolization for OVMS has good clinical effect,which can promote rapid recovery of patients’ life functions,reduce complications,and is safe and effective.
