宁夏居民基层首诊选择及影响因素研究

Study on the Selection of Primary Diagnosis and Influencing Factors among Residents

目的:分析宁夏居民首诊机构选择及其影响因素,为推进基层首诊提出建议。方法:利用第六次全国卫生服务调查宁夏数据库,采用单因素检验和二元logit模型进行分析。结果:个体特征、健康需要、医疗机构和政策环境四类共17个因素对居民基层首诊选择的影响具有显著意义(P<0.05)。结论:重点引导城市居民基层首诊,优化慢性病患者的医保报销策略,改善基层医疗机构的技术和设备,继续推进家庭医生签约服务,从而引导居民基层首诊。

Objective To analyze the reasons and the influencing factors of Ningxia residents'selection of primary diagnosis,and make suggestions for promoting the primary diagnosis.Methods The Ningxia database of the sixth national health service survey was used for analysis by single factor test and binary logistic model.Results A total of 17 factors including individual characteristics,health needs,medical institutions and policy environment had a significant impact on the residents'selection of primary diagnosis(P<0.05).Conclusion Focus on guiding urban residents'primary diagnosis,optimize the medical insurance reimbursement strategy for patients with chronic diseases,improve the technology and equipment of primary medical institutions,continue to promote family doctor contract services,and guide residents to choose primary diagnosis.
