钙卫蛋白结合直肠液渗透压在NEC诊断及病情监测中的应用研究

Application of calprotectin and rectal fluid osmotic pressure in NEC diagnosis and disease monitoring

目的分析钙卫蛋白结合直肠液渗透压在NEC诊断及病情监测中的应用效果。方法选取2019年1月~2020年1月在本院进行诊疗的80例新生儿作为研究对象,以正常同日龄正常新生儿为对照研究对象,将研究对象分成四组:疑似组、确诊组、晚期组和对照组。每组20例患儿。每日动态监测患儿的直肠液渗透压和Fc值,绘制直肠液渗透压和Fc值曲线图,并对直肠液渗透压、Fc值和各期NEC诊断率进行相关性分析。结果相对于对照组,确诊组患儿的钙卫蛋白(67.35±20.94)ng/mL,与直肠液渗透压水平(394.00±11.23)mOsm/kg H2O更高,差异有统计学意义(P<0.05)。钙卫蛋白与直肠液渗透压呈正相关(r=0.612,0.597)。结论在新生儿坏死性小肠结肠炎患儿中,钙卫蛋白与直肠液渗透压的水平更高,且二者存在关联,联合检测可提升疾病诊断的特异性。

Objective To analyze the application effect of calprotectin combined with rectal fluid osmotic pressure in NEC diagnosis and disease monitoring.Methods 80 newborns diagnosed and treated in our hospital from January 2019 to January 2020 were selected as the research subjects,and normal newborns the same days old as the control research subjects.The research subjects were divided into 4 groups with 20 children in each group:suspected group,confirmed group,advanced group and control group.Everyday these children's rectal fluid osmotic pressure and Fc value were monitored dynamically,the corresponding graphs were drawn,and correlation analysis of rectal fluid osmotic pressure,Fc value and NEC diagnosis rate in each phase was conducted.Results Compared with those in the control group,the calprotectin(67.35±20.94)ng/mL and rectal fluid osmotic pressure(394.00±11.23)mOsm/kg H2O in the confirmed group were higher,and the difference was statistically significant(P<0.05).Calprotectin was positively correlated with rectal fluid osmotic pressure(r=0.612,0.597).Conclusion The levels of calprotectin and rectal fluid osmotic pressure are higher in children with NEC,and there is a correlation between the two.Combined detection can improve the specificity of disease diagnosis.
