摘要
目的探讨儿童骨关节感染血清中降钙素原(PCT)与治疗前后体温变化的相关性。方法选择2017年1月至12月于西安市儿童医院骨科收治的骨关节感染患儿62例,男42例,女20例,平均年龄(4.2±0.5)岁,分别记录患儿治疗前体温及PCT值、治疗后体温恢复至正常的时间与治疗后的PCT值,采用Spearman分析PCT与体温、治疗时间的相关性。结果患儿治疗前PCT值的中位数及四分位间距为0.63(0.38~1.77)ng/mL,治疗后为0.05(0.04~0.10)ng/mL,治疗后PCT值显著降低(Z=-9.496,P<0.05);患儿治疗前体温平均为(38.5±0.7)℃,治疗后体温平均为(36.4±0.3)℃,治疗后体温显著降低(t=22.45,P<0.05),患儿治疗前的PCT值与体温存在正相关性(r=0.490,P<0.05);本研究62例患儿经治疗体温恢复至正常所需时间为3(2~5)天,治疗前PCT值与治疗后体温恢复至正常所需时间存在正相关性(r=0.688,P<0.05)。结论 PCT与骨关节感染患儿体温变化具有一定的相关性,其值越高体温恢复至正常所需时间越长,可作为评价其严重程度的指标。
Objective To investigate correlation between serum procalcitonin(PCT) level and temperature change before and after treatment of children with osteoarticular infection.Methods 62 children with osteoarticular infection(including 42 male children and 20 female children aged 18 days to 13 years with an average age of 4.2±0.5 years) who admitted to the department of orthopedics of our hospital from January 2017 to December 2017 were selected.29 patients received surgical treatment and 33 patients received non-operative treatment.Temperature and serum PCT level of the patients at admission,the time when the temperature recovered to normal range and the serum PCT level after treatment were recorded respectively.Spearman correlation analysis was conducted to analyze correlations of the serum PCT level with body temperature and treatment duration.Results Before treatment,the serum PCT level was 0.63(0.38~1.77) ng/mL,after treatment that was 0.05(0.04~0.10) ng/mL.The serum PCT level after treatment was significantly lower than that before treatment(Z=-9.496,P<0.05).The average body temperature of the children was 38.5±0.7℃ before treatment,and that was 36.4±0.3℃ after treatment,and the body temperature significantly lowered after treatment(t=22.45,P<0.05).There was a positive correlation between the serum PCT level and body temperature before treatment(r=0.490,P<0.05).In this study,the time when the body temperature of 62 children recovered to normal range after treatment was 3(2~5) days,and there was a positive correlation between the serum PCT level before treatment and the time when the body temperature recovered to normal range after treatment(r=0.688,P<0.05).Conclusion The serum PCT level has a certain correlation with changes in body temperature of children with osteoarticular infection.The higher the serum level of PCT,the longer the time when the body temperature recovers to normal range.So,it can be used as an indicator to evaluate severity of the disease.
