妇女“两病”筛查的满意度现况调查及供需方因素分析

Surveyon women's satisfaction status of the gynecological and breast diseases screening and analysis of its supply-demand sides factors

目的调查上海市某区"两病"(妇科病和乳腺病)筛查实施过程的满意度现况,从筛查供需方(筛查组织方、体检提供方和参检女性)多角度分析满意度的影响因素。方法采用自拟满意度调查问卷,于2017至2018年在各定点筛查医院通过随机拦截、自愿参加的方式对上海徐汇区13个街道500名参检妇女进行问卷调查。采用多因素二项Logistic回归分析影响因素。结果本次满意度调查对象平均年龄为(59.6±8.10)岁,总体满意度为87.7%,医院服务满意度为80.5%;调查对象对筛查的必要性、有效性和对体检项目了解程度的知晓率分别为82.0%、83.8%和94.2%;调查对象对预约参检的形式、预约过程和到达医院的交通满意度分别为83.0%、85.4%和70.4%。多因素分析显示调查对象对体检项目了解程度、参检形式和预约过程的满意度是影响总体满意度的重要因素,aOR值及95%CI分别为4.155(1.017~16.982)、4.957(1.507~16.310)和7.686(2.917~20.253)。结论可从提高需方的筛查认知,改善供方的筛查预约和候诊过程等方面提升筛查满意度。

Objective To investigate the current status of satisfaction with the implementation of gynecological and breast diseases screening service in a certain district of Shanghai,and analyze the influencing factors of satisfaction from multiple perspectives of screening supply-demand sides(screening organizers,physical examination providers and women).Methods A self-designed satisfaction questionnaire was used to conduct a questionnaire survey on 500 participants from 13 streets of Xuhui District through random interception and voluntary participation in various designated screening hospital from 2017 to 2018.Multivariate binomial Logistic regression was used to analyze the influencing factors.Results The average age of the respondents was 59.6±8.10 years old.The overall satisfaction of screening was 87.7%,and the satisfaction of hospital service provision was 80.5%.Respondents’ awareness rates of the necessity,effectiveness,and knowledge degree of the physical examination items were 82.0%,83.8%,and 94.2%,respectively.Respondents’ satisfaction with the way of appointment,appointment process,and transportation to the hospital were 83.0%,85.4%,and 70.4%,respectively.Multivariate Logistic regression analysis showed that the respondents’ understanding of the screening item,the form of reference examination and the satisfaction with the appointment process were important factors influencing the overall satisfaction of the screening,the aOR values and 95%CIs were 4.155(1.017-16.982),4.957(1.507-16.310) and 7.686(2.917-20.253),respectively.Conclusion Screening satisfaction can be improved in terms of increasing the demand-side screening cognition,improving the supplier’s screening appointment and waiting process.
