电切挂线引流术治疗高位后蹄铁型肛周脓肿临床研究

High Posterior Horseshoe-shaped Perianal Abscess Treated with the Procedure of Electroscission Thread-drawing and Drainage:Clinical Study

为探讨高频电刀切开、挂线、橡皮条引流治疗高位后蹄铁型肛周脓肿的临床效果,将60例高位后蹄铁型肛周脓肿患者随机分为观察组和对照组各30例。对照组采用传统切开挂线术治疗,观察组采用高频电刀切开、挂线、橡皮条引流术治疗。比较两组临床疗效、并发症及后遗症情况。结果显示,观察组术后疼痛时间、出血持续时间、创面愈合时间均短于对照组(P<0.05)。两组有效率均为100%,差异无统计学意义(P>0.05)。观察组复发率为3.33%低于对照组的16.70%(P<0.05),肛门失禁与肛门畸形发生率与对照组比较差异无统计学意义(P>0.05)。结果表明,高频电刀切开、挂线、橡皮条引流治疗高位后蹄铁型肛周脓肿疗效确切,可减轻术后疼痛,缩短疗程,值得临床推广。

This study was to investigate the clinical effect of high posterior horseshoe-shaped perianal abscess(HPHSPA)treated with the combination procedure of electroscission thread-drawing and drainage(with rubber band),randomly divided 60 HPHSPA patients into control group(30 cases,adopted routine incision&thread-drawing)and observation group(30 cases,adopted above compination procedure);then,compared both groups’clinical effect,the status on complication and sequela.As results,in the time of pain lasting,bleeding long-held,and wound-surface healing after surgery observation group was all significantly shorter than control group(P<0.05);the effective rate of both groups was same(100%);in the recurrence rate observation group was lower than control group significantly(3.33%vs 16.67%,P<0.05);in the incidence of anal in continence,and of anal deformity,there was no statistical difference between the two groups(P>0.05).Results show that above-mentioned combination procedure for HPHSPA has certain effect,can reduce postoperative pain,and shorten treatment course,thus is worth to be popularized clinically.
