加减补肺汤联合沙美特罗对慢性阻塞性肺疾病患者肺功能的影响

Effects of Modified Bufei Tang and Salmeterol on Pulmonary Function of COPD Patients

目的:探讨加减补肺汤联合沙美特罗治疗慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)的效果及对患者呼吸指数(respiratory index,RI)、氧合指数(oxygenation index,OI)和动脉-肺泡氧气分率(M指数)的影响。方法:将COPD患者90例采用随机数字表法分为对照组和研究组各45例,对照组采用沙美特罗治疗,研究组采用加减补肺汤联合沙美特罗治疗。观察两组患者疗效、肿瘤坏死因子α(tumor necrosis factorα,TNF-α)和白细胞介素8(interleukin-8,IL-8)水平以及RI、OI和M指数改善情况。结果:治疗后研究组患者第一秒用力呼气容积(first second forced expiratory volume,FEV1)、FEV1%、6 min步行试验(6-minute walk test,6MWT)以及第一秒呼气容积同肺活量(forced vital capacity,FVC)之间的比值(FEV1/FVC)均高于对照组(P<0.05);两组患者TNF-α、IL-8低于对照组(P<0.05);治疗后研究组患者RI和M指数低于对照组,OI高于对照组(P<0.05);有效率研究组为93.4%(42/45),对照组为71.2%(32/45),两组比较差异具有统计学意义(P<0.05)。结论:加减补肺汤联合沙美特罗可以改善COPD患者肺功能,降低血清炎性因子水平,对提高治疗效果以及改善RI、OI和M指数有积极意义。

Objective: To discuss clinical effects of modified Bufei Tang and salmeterol in the treatment of COPD and its influence on RI, OI and M index. Methods: All 90 COPD patients were randomized into the control group and the study group according to random number table method, 45 cases each group, the control group adopted salmeterol, and the study group modified Bufei Tang and salmeterol. To observe clinical effects of both groups, the levels of TNF-α and IL-8, and the improvements of RI, OI and M index. Results: After treating, FEV1, FEV1%,6 MWT and FEV1/FVC of the study group were higher than these of the control group(P<0.05);the levels of TNF-αand IL-8 were lower than these of the control group(P<0.05);after treating, RI and M index of the study group were lower than these of the control group, OI higher than that of the control group(P<0.05);the effective rate of the study group was 93.4%(42/45), higher than 71.2%(32/45) of the control group, and the difference had statistical meaning(P <0.05). Conclusion: Modified Bufei Tang and salmeterol could help improve pulmonary function,relieve the influence of serum inflammatory factors, and it is positive to raise therapeutic effects and improve RI, OI and M index.
