叶氏益肾降糖方治疗早期糖尿病肾病疗效观察

Clinical Effects of Ye’s Yishen Jiangtang Prescription in Treating Early Diabetic Nephropathy

目的:观察叶氏益肾降糖方治疗早期糖尿病肾病的临床疗效。方法:将早期糖尿病肾病患者74例随机分两组各37例,对照组采用保肾、降糖、降压、改善微循环治疗;观察组在对照组基础上采用叶氏益肾降糖方治疗,持续8周。比较两组患者临床疗效、尿微量白蛋白(microalbuminuria,mAlb)、尿中白蛋白/肌酐(urinary albumin-to-creatinine ratio,UACR)、肾小球滤过率(estimated glomerular filtration rate,eGFR)及血清白细胞介素6(interleukin-6,IL-6)水平,并记录不良反应。结果:有效率对照组为78.4%(29/37),低于观察组的94.6%(35/37),差异具有统计学意义(P<0.05)。治疗后对照组和观察组UACR分别为(16.25±4.56)mg/mmol、(10.57±4.21)mg/mmol,mAlb分别为(75.20±13.36)mg/L、(53.31±12.20)mg/L,e GFR分别为(48.02±4.65)mL/min、(52.29±4.72)mL/min,血清IL-6分别为(17.49±4.91)ng/L、(12.68±4.56)ng/L,两组各指标比较差异具有统计学意义(P<0.05)。两组患者治疗过程中均未见不良反应。结论:在西医常规治疗的基础上应用叶氏益肾降糖方有利于降低早期糖尿病肾病患者炎症因子,避免肾损伤,临床疗效优于单用西医常规治疗。

Objective: To observe clinical effects of Ye′s Yishen Jiangtang prescription in the treatment of early diabetic nephropathy(DN). Methods: All 74 patients were randomized into two groups, 37 cases each group,the control group adopted the therapy of protecting kidney, lowering glucose, lowering pressure and improving microcirculation;and the observation group took Ye′ s Yishen Jiangtang prescription orally on the foundation of Western medicine, for eight consecutive weeks. To compare clinical effects, m Alb, UACR, eGFR and IL-6 between both groups, to record adverse reaction. Results: The effective rate of the control group was 78.4%(29/37), lower than 94.6%(35/37) of the observation group, and the difference had statistical meaning(P <0.05). After treating,UACR of the control group and the observation group were(16.25 ±4.56)mg/mmol and(10.57 ±4.21)mg/mmol respectively, m Alb(75.20±13.36)mg/L and(53.31±12.20)mg/L, eGFR(48.02±4.65)mL/min and(52.29±4.72)mL/min,IL-6(17.49±4.91)ng/L and(12.68±4.56)ng/L, the difference had statistical meaning between both groups in these indexes(P <0.05). No adverse reaction happened in both groups. Conclusion: Ye′ s Yishen Jiangtang prescription could reduce inflammatory factors, avoid renal damage with definited clinical effects in the treatment of DN.
