塞来昔布胶囊联合中药熏洗防治早期膝骨性关节炎临床观察

Clinical Observation on Celecoxib Capsules Joined with TCM Fumigation in the Prevention and Treatment of Early Knee Osteoarthritis

目的:观察塞来昔布胶囊联合中药熏洗防治早期膝骨性关节炎的效果。方法:将早期膝骨性关节炎患者83例根据入院顺序随机分为研究组41例和对照组42例,对照组给予塞来昔布胶囊治疗,研究组在对照组基础上联合中药熏洗治疗,对比两组患者的临床疗效、关节液炎症因子水平变化情况、膝关节功能和疼痛评分。结果:有效率研究组为90.2%(37/41),高于对照组的76.2%(32/42)(P<0.05);治疗后两组患者疼痛评分均降低,膝关节功能评分均升高,且研究组改善更显著(P<0.05);治疗后两组患者基质金属蛋白酶3(matrix metalloproteinase 3,MMP-3)、肿瘤坏死因子α(tumor necrosis factorα,TNF-α)、白细胞介素1β(interleukin-1β,IL-1β)水平均较治疗前降低,且研究组降低更显著(P<0.05)。结论:塞来昔布胶囊联合中药熏洗可有效调节早期膝骨性关节炎患者膝关节液炎症因子水平,改善膝关节功能,降低疼痛程度,临床效果优于予塞来昔布胶囊。

Objective: To observe clinical effects of celecoxib capsules in combination with TCM fumigation in the prevention and treatment of early knee osteoarthritis(KOA). Methods: All 83 patients were randomized into 41 cases of the study group and 42 cases of the control group according to the admission order, the control group were treated by celecoxib capsules, and the study group accepted TCM fumigation based on Western medicine the control group accepted, to compare clinical effects, the changes of inflammatory factors in synovial fluid, knee function and pain scores between both groups. Results: The effective rate of the study group was 90.2%(37/41), higher than 76.2%(32/42) of the control group(P <0.05);after treating, pain scores reduced, and knee joint functional scores rose, and the improvements of the study group were more notable(P <0.05);after treating, the levels of MMP-3, TNF-α and IL-β in both groups were lower compared with these before treating, and the decrease of the study group was more obvious(P <0.05). Conclusion: Celecoxib capsules in combination with TCM fumigation could effectively regulate the levels of inflammatory factors in synovial fluid of KOA patients, improve knee joint function, reduce pain degrees, and clinical effects superior to these of celecoxib capsules.
