医联体医院患者就医行为心理归因调查与分析

Investigation and analysis on the psychological attribution of patients’medical treatment behavior under municipal medical conjoined hospital

目的:调研医联体模式下患者就医行为心理归因对分级诊疗运行的影响,为深入推进医联体模式下分级诊疗提供参考。方法:选取某市医联体内2家核心医院及6家联合医院,通过自行设计问卷,从就医心理角度对就诊患者进行面对面问卷调研,同时进行专题访谈。结果:59%的患者对医联体模式尚不知晓,36%的患者表示知道医联体模式,完全了解者仅占5%。居民就医首选社区卫生服务中心或县级医院等基层医院人数占比为35%。结论:患者就医心理易受年龄、学历背景、就医观念、消费观念和心理承受力等多重因素的影响,心理因素产生的就医选择动机偏差是影响分级诊疗的重要因素,对全民就医理念进行深入宣传与普及性教育,纠正患者心理归因偏差对推进医联体制度实施具有重要意义。

Objective:This article analyzes the influence of psychological attribution on the implementation of graded diagnosis and treatment under the mode of medical alliance in order to provide reference for further improvement of the policy of graded diagnosis and treatment in medical alliance.Methods:Two core hospitals and six united hospitals in a city medical union were randomly selected as research places.Through self-designed questionnaire,face-to-face questionnaire survey was carried out on patients from the psychological point view of patients and related interviews were carried out at the same time.Results:59%of the patients did not know the medical union model,36%of the patients said they knew the medical alliance but only 5%knew it very well.The proportion of patients who first choose community health service center or county-level hospital and other grass-roots hospitals was 35%.Conclusion:Patients’psychology is easily affected by many factors,such as age,education level,medical concept and psychological tolerance.The motivation of medical choice induced by psychological factors deviating from the policy of hierarchical diagnosis and treatment exerted major influence.In-depth publicity and universal education of the concept of medical treatment for the whole people,and correcting the deviation of patients’psychological attribution are of great significance to the implementation of the medical consortium system.
