髓内占位切除术中脊髓后正中沟定位技术的改良及应用效果分析(附四例报道) 被引量:1

摘要 髓内占位切除术通常需沿着脊髓后正中沟切开脊髓.以防止脊髓背索功能受损,确保手术人路的安全。脊髓后正中沟通常可以通过背正中沟静脉以及两侧神经根中点等标志来识别。然而,髓内肿瘤、囊肿或积液的压迫可能导致脊髓局部发生推挤、旋转,脊髓后正中沟的解剖位置发生扭曲或者是水肿、表面新生血管、局部瘢痕等形成,使术者很难通过肉眼准确辨别脊髓后正中沟的位置。曾有研究报道,在髓内占位患者中,仅凭肉眼和经验辨别脊髓后正中沟的位置,可能会有40%的误判率。而手术中若沿着错判的脊髓后正中沟行脊髓切开.势必会损伤脊髓背索,导致患者术后深感觉功能丧失。
