医院中与医疗器械相关的血液感染影响因素分析 被引量:2

摘要 目的探讨医院中与医疗器械相关的血液感染发生的影响因素。方法选取2011年4月—2017年4月北京市某三级综合医院发生与医疗器械相关的血液感染患者100例作为研究组,另选取该院同期未发生医疗器械相关的血液感染患者100例作为对照组。分别收集两组100组医疗器械、100次空气生物检测数据以及100例医务人员的卫生生物进行检测。对比两组医疗器械清洗消毒合格率、空气生物检测合格率以及医务人员手卫生合格率。采用Logistic回归,分析医院中与医疗器械相关的血液感染的影响因素。结果对照组肉眼裸视法与放大镜法下医疗器械清洗消毒合格率分别为95.00%、90.00%,均显著高于研究组(66.00%、54.00%,均P<0.05)。对照组空气生物检测合格率为83.00%,明显高于研究组(47.00%,P<0.05)。对照组医务人员手卫生合格率为96.00%,显著高于研究组(53.00%,P<0.05)。Logistic回归分析显示,医疗器械清洗消毒不合格、空气生物检测不合格及医务人员手卫生生物检测不合格是影响医院中与医疗器械相关的血液感染的独立危险因素(均P<0.05)。结论医院中与医疗器械相关的血液感染发生的危险因素主要包括医疗器械清洗消毒不合格、空气生物检测及医务人员手卫生生物检测不合格,在临床工作中可通过针对性措施干预,降低医疗器械相关血液感染的发生风险。 Objective To study the risk factors of nosocomial blood infection related to medical devices.MethodsFrom April 2011 to April 2017,100 cases of blood infection related to medical device from Beijing Civil Aviation General Hospital were selected as study group.During the same period,100 cases of patients without blood infection in this hospital were taken as the control group.100 sets of medical instruments,100 times of air biological monitoring data and 100 cases of health biological monitoring of medical staff in two groups were collected respectively.Medical appliance cleaning and disinfection qualified rate,air qualified rate of biological monitoring and medical staff hand hygiene rate were compared,and Logistic regression was performed to analyze the related factors of blood infection related to hospital medical devices.Results The magnifying glass bare naked eye under medical instrument cleaning and disinfection qualified rate were 95%(95/100),90%(90/100)in the control group,which were significantly higher than the study group of 66%(66/100),54%(54/100),and there were significant difference(P<0.05).The qualified rate of air bioassay in the control group was 83%(83/100),which was significantly higher than the study group of 47%(47/100)with statistically significant(P<0.05).The qualified rate of hand hygiene of medical staff in the control group was 96%(96/100),which was significantly higher than the study group of 53%(53/100)with statistically significant(P<0.05).Logistic regression analysis showed that medical equipment cleaning,the air biological test and the hand hygiene biological monitoring of medical personnel were the independent risk factors of blood infection related to medical device(P<0.05).Conclusion The risk factors of hospital infection related medical devices include medical device cleaning disinfection unqualified,air biological testing unqualified and medical personnel hand hygiene biological monitoring unqualified.The risk of blood infections related medical device can be reduced through targeted intervention in the clinical work.
