正畸联合修复对牙列缺损伴牙颌畸形患者咬合功能影响

Effect of combined orthodontic repair on occlusal function in patients with dentition defect and dental maxillofacial deformity

目的牙列缺损伴牙颌畸形不仅影响患者咀嚼功能,同时影响其外貌美观,给患者日常生活造成严重影响。本研究探讨正畸联合修复牙列缺损伴牙颌畸形患者咬合功能的影响。方法选择2017-06-01-2019-01-31洛宜路三好口腔门诊部收治的84例牙列缺损伴牙颌畸形患者作为研究对象,根据两组患者一般资料具有可比性的原则分为对照组和观察组,各42例。对照组采用单纯修复疗法治疗,观察组采用正畸联合修复疗法治疗,对比两组临床疗效和咬合功能。结果观察组治疗优良率为92.86%,高于对照组的76.19%,χ^2=4.459,P=0.035。治疗后,观察组最大咬合接触面积为(218.64±36.38)mm2,高于对照组的(176.72±28.56)mm2,t=5.874,P<0.001;观察组最大咬合力为(25.42±8.35)kg,高于对照组的(17.61±7.13)kg,t=4.610,P<0.001;观察组咬合力不对称指数为15.27±8.23,低于对照组的23.68±10.39,t=4.112,P<0.001。观察组牙齿排列整齐率为100.00%,高于对照组的73.81%,χ^2=12.658,P<0.001;观察组后牙咬合良好率为95.24%,高于对照组的76.19%,χ^2=6.222,P=0.013;观察组前牙覆颌与覆盖良好率为97.62%,高于对照组的78.57%,χ^2=7.265,P=0.007。结论对牙列缺损伴牙颌畸形患者采用正畸联合修复疗法有助于矫正患者牙齿,提升咬合功能,改善牙齿美观,效果高于单纯修复治疗。

OBJECTIVE Dentition defect with dental jaw deformity not only affects patients’chewing function,but also affects their beautiful appearance,which seriously affects patients’daily lives.This study investigated the effect of orthodontic repair on dentition defect with occlusal function in patients with dentition.METHODS A total of 84 patients with dentition defects and jaw deformities treated in Sanhao Dental Clinic,Luoyi Road were selected from June 1,2017 to January 31,2019.According to the principle of comparability between the two groups of patients,the patients were divided into the control group(42 cases)and the observation group(42 cases).The control group was treated with simple repair therapy,the observation group was treated with orthodontic combined repair therapy,and the efficacy and occlusal function of the two groups were compared.RESULTS The excellent and good treatment rate in the observation group was 92.86%,which was higher than 76.19%in the control group,χ^2=4.459,P=0.035.After treatment,the maximum occlusal contact area in the observation group was(218.64±36.38)mm2,which was higher than the control group(176.72±28.56)mm2,t=5.874,P<0.001.The maximum occlusal force in the observation group was(25.42±8.35)kg,higher than the control group(17.61±7.13)kg,t=4.610,P<0.001.The asymmetry index of bite force in the observation group was 15.27±8.23,lower than that in the control group 23.68±10.39,t=4.112,P<0.001.The teeth alignment rate in the observation group was 100.00%,which was higher than 73.81%in the control group,χ^2=12.658,P<0.001.The rate of good teeth occlusion in the observation group was 95.24%,which was higher than76.19%in the control group,χ^2=6.222,P=0.013.The good rate of anterior teeth maxilla and coverage in the observation group was 97.62%,which was higher than 78.57%in the control group,χ^2=7.265,P=0.007.CONCLUSION The use of orthodontic combined repair therapy for patients with dentition defect and dental jaw deformity can help to correct the patient’s teeth,improve occlusal function,and improve the aesthetics of the teeth.
