低离子凝聚胺与盐水法交叉配血试验效果比较

Comparison of cross-matching test between low-ion polybrene and saline method

摘要 目的输血环节较多,极易出现配血错误,导致不良输血反应的发生,轻者影响疾病治疗,重者威胁患者生命健康。本研究对比低离子凝聚胺与盐水法交叉配血试验对输血检验结果的影响。方法收集2018-01-01-2019-02-01南阳市第二人民医院接受输血治疗的63例患者样本,分别进行低分子凝聚胺与盐水交叉配血试验。比较两种检测方法的检测时间;观察两种方法进行配血试验的细胞凝结检出情况;比较两组检测方法进行交叉配血试验的灵敏度及准确性。结果低分子凝聚胺法检测时间为(2.58±0.27)min,短于盐水法的(4.93±0.54)min,t=30.895,P<0.001。低分子凝聚胺法进行交叉配血试验的阳性检出率为15.87%,高于盐水法的4.76%,χ^2=4.203,P<0.001;灵敏度为90.00%,高于盐水法的20.00%,χ^2=9.899,P=0.002;准确度为95.24%,高于盐水法的84.13%,χ^2=4.203,P=0.040;两种方法检测特异度对比差异无统计学意义,P>0.05。低分子凝聚胺法进行配血试验时肉眼可见细胞凝集,而盐水法需在显微镜下才能看见细胞凝集。结论采用低分子凝聚胺进行交叉配血试验能缩短检测时间,提高灵敏度和稳定性。 OBJECTIVE There are many blood transfusion links,and it is easy to cause blood transfusion errors,which will lead to the occurrence of adverse transfusion reactions.The mild ones will affect the treatment of the disease and the serious ones will threaten the patient’s life and health.This study compared the effects of low-ion polybrene and saline cross-matching tests on the results of blood transfusion tests.METHODS A sample of 63 patients who received blood transfusion treatment from January 1,2018 to February 1,2019 in Nanyang Second People’s Hospital was collected,and a low-molecular-weight polybrene and saline cross-matching test was performed.The detection time of the two detection methods was compared.The cell coagulation detection of the two methods for the blood matching test was observed.The sensitivity and accuracy of the two methods for the cross blood matching test was compared.RESULTS The detection time of the low-molecular-weight polybrene method was(2.58±0.27)min,which was shorter than that of the saline method(4.93±0.54)min,t=30.895,P<0.001.The positive detection rate of cross-matching test with low molecular weight polybrene method was 15.87%,which was higher than 4.76%of saline method,χ^2=4.203,P<0.001.The sensitivity was 90.00%,which was higher than 20.00%of saline method,χ^2=9.899,P=0.002.The accuracy was95.24%,higher than 84.13%of the saline method,χ^2=4.203,P=0.040.There was no statistically significant difference in the specificity of the two methods,P>0.05.Low-molecular-weight agglutination method can be used to visualize cell agglutination when blood matching test is performed,while saline method requires a microscope to see cell agglutination.CONCLUSION The cross-matching test using low-molecular-weight polybrene can shorten the detection time,improve sensitivity and stability.
