自拟润肤祛风汤结合西医常规疗法治疗慢性湿疹血虚风燥证临床研究

Clinical study of self-made Runfu-Qufeng Decoction combined with conventional western medicine in the treatment of chronic eczema with blood deficiency and wind dryness syndrom

目的评价自拟润肤祛风汤结合西医常规疗法治疗慢性湿疹血虚风燥证患者的临床疗效。方法选择2017年1月-2018年10月符合入选标准的本院患者80例,按随机数字表法分为2组,每组40例。对照组采用西医常规疗法治疗,观察组在对照组基础上加服自拟润肤祛风汤。2组均连续治疗30 d,随访3个月。分别于治疗前后进行临床症状评分,采用ELISA法检测血清IL-4、γ干扰素(interferon-γ,INF-γ)含量;评价临床疗效,记录复发情况。结果观察组总有效率为95.0%(38/40)、对照组为80.0%(32/40),2组比较差异有统计学意义(χ^2=4.114,P=0.043)。观察组治疗后瘙痒程度、皮损面积、皮疹形态评分及总分均低于对照组(t值为9.538、4.383、7.992、4.094,P值<0.001)。治疗后,观察组血清IL-4水平[(42.05±3.84)ng/L比(47.29±4.16)ng/L,t=5.854]低于对照组(P<0.01),INF-γ[(80.13±7.10)ng/L比(74.04±6.26)ng/L,t=4.069]水平高于对照组(P<0.01)。随访3个月,对照组复发率为21.9%(7/32)、观察组为2.6%(1/38),2组比较差异有统计学意义(χ^2=5.000,P=0.025)。结论自拟润肤祛风汤结合西医常规疗法可改善慢性湿疹血虚风燥证患者的临床症状,提高临床疗效。

Objective To evaluate the clinical effect of self-made Runfu-Qufeng Decoction combined with conventional western medicine in the treatment of chronic eczema with blood deficiency and wind dryness syndrom.Methods A total of 80 patients in our hospital from January 2017 to October 2018,who met the inclusion criteria,were selected and divided into two groups according to the random table method,40 in each group.The control group was treated with conventional western medicine,while the research group was treated with self-made Runfu-Qufeng Decoction on the basis of control group.Both of the two groups were treated for 30 consecutive days with 3 months follow-up.The clinical symptom was scored before and after the treatment and the serum interleukin-4(IL-4)as well asinterferon-γ(INF-γ)were detected by ELISA.The clinical efficacy was evaluated and the recurrence was recored.Results The total effective rate of research group was 95.0%(38/40),while the control group was 80.0%(32/40).The difference between the two groups was statistically significant(χ^2=4.114,P=0.043).After the treatment,scores of pruritus degree,lesion area,rash morphology and total scores in the research group were significantly lower than those of the control group(t values were 9.538,4.383,7.992,4.094,all Ps<0.001).After the treatment,the serum IL-4 level(42.05±3.84 ng/L vs.47.29±4.16 ng/L,t=5.854)of research group was significantly lower than that of the control group(P<0.01),while the INF-γ(80.13±7.10 ng/L vs.74.04±6.26ng/L,t=4.069)was significantly higher than that of the control group(P<0.01).After 3 months of follow-up,the recurrence rate of the control group was 21.9%(7/32)and that of the research group was 2.6%(1/38).The difference between the two groups was statistically significant(χ^2=5.000,P=0.025).Conclusions The self-made Runfu-Qufeng Decoction combined with conventional western medicine could relieve the clinical symptoms of patients with chronic eczema due to blood deficiency and wind dryness syndrom and improve the clinical efficacy.
