鹿茸方治疗糖尿病神经源性膀胱随机平行对照研究

Randomized Controlled Study on Lurong Formula for Diabetic Neurogenic Bladder

目的观察以补肾活血法立方的鹿茸方治疗肾虚血瘀型糖尿病神经源性膀胱的疗效。为临床提供一个治疗糖尿病神经源性膀胱的新方法。方法使用随机平行对照研究方法,将64例患者按照入组顺序,随机分为两组对照组和治疗组。对照组32例,在糖尿病生活方式干预的前提之下,选择口服药物或胰岛素皮下注射将血糖控制在理想范围,给予甲钴胺片,500μg/次,3次/d,口服。治疗组32例,在对照组治疗基础之上,给予具有补肾活血作用的鹿茸方。连续用药治疗8周。治疗结束后,观察中医证候变化、B超膀胱残余尿量、最大尿流率、血糖、糖化血红蛋白、不良反应。结果治疗组中显效6例,有效23例,无效3例,总有效率90.6%(29/32)。对照组中显效4例,有效16例,无效12例,总有效率62.5%(20/32)。总有效率比较显示,治疗组疗效优于对照组(P<0.05)。中医证候积分比较显示,治疗组改善优于对照组(P<0.05)。膀胱残余尿量、最大尿流率比较显示,治疗组优于对照组(P<0.05)。结论补肾活血法组方的鹿茸方治疗肾虚血瘀型糖尿病神经源性膀胱,疗效满意,无严重不良反应,值得推广。

Objective To observe the clinical curative effect of Lurong Formula which can tonify kidney and activate blood circulation for neurogenic bladder of diabetes with kidney deficiency and blood stasis syndrome and provide a new treatment for diabetic neurogenic bladder.Methods By randomized controlled method,64 patients were randomly divided into two groups by sequencing.They were all given perfectly blood sugar control by using medicine or insulin.The control group was treated with mecobalamin tablets.The treatment group used Lurong Formula and mecobalamin tablets.They were treated for eight weeks to determine efficacy.After the course,we observed the changes of TCM syndromes and residual urine volume of bladder and maximum urine flow rate.Result In the treatment group,6 cases were markedly effective,23 cases effective,3 cases ineffective and the total effective rate was 90.6%(29/32).In the control group,4 cases were markedly effective,16 cases effective,12 cases ineffective and the total effective rate was 62.5%(20/32).The curative effect of the treatment group was better than that of the control group(P<0.05).The scores of TCM syndromes in both groups were significantly decreased(P<0.01)and the decrease in the treatment group was better than that in the control group(P<0.01).Conclusion Lurong Formula is effective on treating diabetic neurogenic bladder and has no serious adverse reactions.It is worthy of popularizing.
