醒脑静注射液结合神经生长因子对老年重型颅脑损伤预后及血清MBP、NSE的影响 被引量:18

Effect of Xingnaojing Injection Combined with Nerve Growth Factor on Prognosis and Serum MBP and NSE in Elderly Patients with Severe Craniocerebral Injury

摘要 目的探讨醒脑静注射液结合神经生长因子在老年重型颅脑损伤患者治疗中的应用效果。方法将2016年8月—2018年8月收治的92例老年重型颅脑损伤患以随机数字表法分为对照组46例、观察组46例,对照组给予神经生长因子,观察组在对照组基础上加用醒脑静注射液。观察两组治疗前、治疗2周后病情及血清髓鞘碱性蛋白(MBP)、神经元特异性烯醇化酶(NSE)水平变化,住院期间并发症发生率及伤后12周预后情况。结果治疗前,两组病情无明显差异(P>0.05),治疗2周后,观察组格拉斯哥昏迷量表(GCS)评分与同期对照组对比,明显较高,美国国立卫生研究院卒中量表(NIHSS)及急性生理与慢性健康评分量表(APACHE-Ⅱ)评分与同期对照组对比,明显较低(P<0.05);治疗前,两组血清MBP、NSE水平无明显差异(P>0.05),治疗2周后,观察组血清MBP、NSE水平与同期对照组对比,明显较低(P<0.05);观察组住院期间并发症发生率与对照组对比,明显较低(P<0.05);观察组伤后12周预后良好率为82.61%(38/46),与对照组63.04%(29/46)对比,明显较高(P<0.05)。结论醒脑静注射液结合神经生长因子治疗老年重型颅脑损伤,可有效缓解病情,降低血清MBP、NSE水平,改善预后,值得推广。 Objective To investigate the effect of Xingnaojing Injection combined with nerve growth factor in the treatment of elderly patients with severe craniocerebral injury.Methods Ninety-two elderly patients with severe craniocerebral injury admitted from Aug 2016 to Aug 2018 were randomly divided into the control group(46 cases)and the observation group(46 cases).The control group was given nerve growth factor,and the observation group was given Xingnaojing Injection on the basis of the control group.We observed the changes of condition and serum levels of myelin basic protein(MBP)and neuron-specific enolase(NSE)before and after 2 weeks of treatment,the incidence of complications during hospitalization and the prognosis of 12 weeks after injury of two groups.Results Before treatment,there was no significant difference in condition between the two groups(P>0.05).After 2 weeks of treatment,the Glasgow Coma Scale(GCS)score of the observation group was significantly higher compared with that of the control group at same time.The national institutes of health stroke scale(NIHSS)and the acute physiology and chronic health evaluation scale(APACHE-II)score were significantly lower compared with those of the control group at same time(P<0.05).Before treatment,there was no significant difference in serum levels of MBP or NSE between the two groups(P>0.05).After 2 weeks of treatment,the serum levels MBP or NSE in the observation group were significantly lower compared with those of the control group at same time(P<0.05).The incidence of complications during hospitalization in the observation group was significantly lower compared with that of the control group(P<0.05).The prognosis rate of 12 weeks after injury was 82.61%(38/46),significantly higher than 63.04%(29/46)in the control group(P<0.05).Conclusion Xingnaojing Injection combined with nerve growth factor can effectively improve the condition of senile severe craniocerebral injury,reduce the serum levels of MBP and NSE,and improve the prognosis.It is worthy of promoting.
