新生儿细菌性感染治疗前后血清PCT、hs-CRP及WBC水平变化及临床意义 被引量:14

Changes in the Levels of Serum PCT,hs-CRP and WBC before and after treatment of Neonatal Bacterial Infection and Their Clinical Significance

摘要 目的探讨细菌性感染新生儿血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)及白细胞(WBC)水平检测的临床价值。方法回顾性分析我院2017年10月—2018年9月收治的124例细菌性感染新生儿的临床资料,比较治疗前后及不同分组患儿血清PCT、hs-CRP及WBC变化,分析血清PCT、hs-CRP及WBC水平与疗效的相关性。结果124例细菌性感染患儿治疗24、72 h血清PCT、hs-CRP及WBC水平较治疗前显著降低,且治疗72 h低于治疗24 h(P<0.05)。局部感染组和普通感染组治疗前血清PCT、hs-CRP及WBC水平均显著低于全身感染组和重症感染组(P<0.05)。革兰阳性菌组血清PCT水平低于革兰阴性菌组,WBC水平显著高于革兰阴性菌组(P<0.05),但2组血清hs-CRP水平比较差异无统计学意义(P>0.05)。血清PCT、hs-CRP及WBC水平均与细菌性感染患儿的疗效呈显著负相关(r=-0.761、-0.704、-0.735,P<0.05)。结论新生儿细菌性感染治疗前后进行血清PCT、hs-CRP及WBC检测可在一定程度反映病情进展与疗效,对预后评估有积极意义。 Objective To explore the clinical value of serum procalcitonin(PCT),hypersensitive-C-reactive protein(hs-CRP)and white blood cell count(WBC)levels in neonates with bacterial infection.Methods The clinical data of 124 neonates with bacterial infection admitted to our hospital from October 2017 to September 2018 were analyzed retrospectively.The changes of serum PCT,hs-CRP and WBC levels before and after treatment and in different groups were compared.The correlation between the levels of serum PCT,hs-CRP and WBC and the therapeutic effect was analyzed.Results The levels of serum PCT,hs-CRP and WBC in 124 children with bacterial infection at 24 and 72 h after treatment were significantly lower than those before treatment,and the indicators at 72 h after treatment were lower than those at 24 h after treatment(P<0.05).The levels of PCT,hs-CRP and WBC in local infection group and common infection group were significantly lower than those in systemic infection group and severe infection group(P<0.05).In the Gram-positive group,level of serum PCT was significantly lower,and WBC was significantly higher than those in the Gram-negative group(P<0.05),but there was no significant difference in serum hs-CRP level between the two groups(P>0.05).Serum levels of PCT,hs-CRP and WBC were significantly negative correlation with curative effects of neonates with bacterial infection(r=-0.761,-0.704 and-0.735,P<0.05).Conclusion The detection of serum PCT,hs-CRP and WBC levels before and after treatment of neonatal bacterial infection can reflect the progression of disease and the therapeutic effects to a certain extent,which is of positive significance for prognosis evaluation.
