新型冠状病毒肺炎隔离病房床旁超声检查的实践与总结

Summary and implementation of bedside ultrasound examination in isolation ward of COVID-19

目的探讨床旁超声在新型冠状病毒肺炎(以下简称新冠肺炎)隔离病房内的应用价值及实施方法。方法统计分析2020年2~3月同济医院光谷院区新冠肺炎隔离病房内床旁超声的检查项目、数量、时间、诊断、防护方法及具体实施策略。结果共完成450人次床旁超声检查。超声医师平均每天进入隔离病房2.3次,每次耗时2.9 h。主要阳性发现为心脏异常、深静脉血栓、腹部脏器异常及腹水等。在二级及三级防护条件下,床旁超声检查的医护人员均未出现新型冠状病毒感染。结论床旁超声在新冠肺炎的诊疗过程中发挥重要作用,但实施难度加大、感染风险高,需不断总结实践经验,制定安全、高效的实施策略。

ObjectiveTo explore the application value and detailed implementation strategies of bedside ultrasoundin the diagnosis and treatment of COVID-19 patients in isolation ward.MethodsStatistical analysis of the items,quantity,duration,diagnosis,protection methods and detailed implementation strategies of bedside ultrasound examinations in COVID-19 isolation ward in Guanggu district of Tongji Hospital from February to March 2020.ResultsA total of 450 times bedsideultrasound examinations were performed.The sonographers entered the isolation ward 2.3 times per day,each time took 2.9 h onaverage.The mainly positive findings of bedside ultrasound examinations included cardiac abnormalities,deep vein thrombosis,abdominal viscera abnormalities and peritoneal effusion,etc.No COVID-19 infection occurred in the sonographers under level-Ⅱ and-Ⅲ standard protections.ConclusionBedside ultrasound examination plays an important role in the diagnosis andtreatment of the COVID-19 patients in isolation ward.However,due to the increased difficulty in implementation and high riskof COVID-19 infection,it’s necessary to constantly summarize practical experience and formulate safe and efficientimplementation strategies.
