个性化护理对老年腹股沟患者无张力疝修补术围手术期并发症和护理满意度的影响

Effect of individualized nursing on the incidence of perioperative complications and nursing satisfaction after tension-free inguinal hernia repair in elderly patients

目的探究个性化护理对老年腹股沟无张力疝修补术后并发症发生率和护理满意度的影响。方法选择2018年4月至2019年8月,海南西部中心医院接受无张力疝修补术治疗的老年腹股沟疝患者90例。通过随机数字表法将患者分为观察组(45例)及对照组(45例)。对照组采用常规护理,观察组接受个性化护理,2组均护理至患者出院。比较2组护理效果、疼痛视觉模拟评分(VAS)、生命质量(QOL)评分、术后并发症发生率以及护理满意度。结果观察组手术时间、下床活动时间、肛门排气时间、住院天数均短于对照组(P<0.05)。出院时观察组VAS评分低于对照组,QOL评分、护理满意度评分均高于对照组(P<0.05)。观察组围手术期并发症发生率为6.67%(3/45),显著低于对照组的26.67%(12/45)(P<0.05)。结论个性化护理在腹股沟无张力疝修补术后老年患者中应用效果良好,可改善患者手术相关指标,减少围手术期并发症的发生,减轻患者疼痛,进而提高其生活质量,获得患者认可。

Objective To analyze the effect of individualized nursing on the incidence of complications and nursing satisfaction after tension-free inguinal hernia repair in elderly patients.Methods Between April 2018 and August 2019,90 elderly patients with inguinal hernia underwent tension-free hernia repair in Hainan western central hospital were selected.The patients were randomly divided into observation group(n=45)and control group(n=45).The control group was treated with routine nursing,the observation group received individualized nursing,and both groups were cared until the patient was discharged from hospital.The nursing effect,clinical efficacy,scores of visual analogue scale(VAS)and quality of life(QOL),postoperative complications and nursing satisfaction were compared between the two groups.Results The operation time,the time of getting out of bed,the time of anal exsufflation and the stay of hospitalization in the observation group were less than those in the control group(P<0.05).At discharge,the VAS score of the observation group was lower than that of the control group,and the QOL score and nursing satisfaction score of the observation group were higher than those of the control group(P<0.05).The incidence of perioperative complications in the observation group was 6.67%(3/45),which was significantly lower than that in the control group[26.67%(12/45),P<0.05].Conclusion Individualized nursing is effective in elderly patients with tension-free inguinal hernia repair.It can improve the surgical related indexes,reduce the occurrence of perioperative complications,reduce the pain of patients,and then promote the postoperative recovery of patients,improve their quality of life,and obtain the approval of patients.
