骨小梁假体与S-ROM假体在初次全髋关节置换术中的应用比较 被引量:2

摘要 目的探讨骨小梁假体与S-ROM假体在初次全髋关节置换术(total hip arthroplasty,THA)中的应用效果。方法2012年10月~2018年10月初次行THA治疗的76例患者按随机数字表法分为两组:A组38例,采用骨小梁假体;B组38例,采用S-ROM假体。观察并比较两组手术指标、术后疗效,评估术前、术后1周、1个月、6个月疼痛VAS评分、髋关节功能及并发症情况。结果两组手术指标比较,A组手术时间、术中出血量均少于B组,差异有统计学意义(P<0.05);而两组手术切口长度、下床活动时间、住院时间比较,差异均无统计学意义(P>0.05)。术后1周A组VAS评分高于B组(P<0.05),但两组患者术前、术后1个月及术后6个月疼痛VAS评分比较,差异无统计学意义(P>0.05);两组术前、术后1周、术后1个月、术后6个月髋关节Harris评分比较,差异均无统计学意义(P>0.05)。术后12个月A组髋关节功能恢复优良率为92.11%,B组为94.74%,两组比较差异无统计学意义(P>0.05)。A组并发症发生率为5.26%,B组为10.53%,组间比较差异无统计学意义(P>0.05)。结论在初次行THA的患者中,采用骨小梁假体与S-ROM假体可获得相似的近期疗效,但前者手术时间更短,出血更少,后者可减轻术后短期疼痛,应根据患者具体情况予以选择。 Objective To investigate the application effect of trabecular bone prosthesis and S-ROM prosthesis in the first total hip arthroplasty(THA).Methods A total of 76 patients who were first treated by THA from October 2012 to October 2018 were divided into two groups by random number table method,with 38 cases for each group.Trabecular bone prosthesis was applied in group A,and S-ROM prosthesis in group B.The surgical indexes,the postoperative effects,the VAS score of pain,hip function before the operation,1 week-,1 month-and 6 months after operation and complications of two groups were observed.Results The operation time was shorter and the intra-operative blood loss volume was less in the group A than those in the group B(P<0.05),but there were no significant difference in incision length,ambulation time and hospitalization time between the two groups(P>0.05).Except the VAS score of 1 week after the operation was higher in the group A than the group B(P<0.05),there was no the significant difference in VAS score before the operation,1-and 6 months after the operation and in Harris score before the operation,1 week,1-and 6 months after the operation between group A and B(P>0.05).The excellent and good rate of hip joint function recovery was 92.11%in group A and 94.74%in group B at 12 months after the surgery,and no statistical difference between the two groups(P>0.05).The complication rate was 5.26%and 10.53%in group A and group B respectively,and no statistical difference was found between them(P>0.05).Conclusion In the patients with THA for the first time,trabecular bone prosthesis and S-ROM prosthesis can achieve similar short-term effects,but the former has shorter operation time and less bleeding,the latter can reduce short-term postoperative pain,and the prosthesis can be selected according to the specific circumstances of the patients.
