心理弹性在胶质瘤患者领悟社会支持与癌症复发恐惧间的中介效应

Mediating effect of resilience on perceived social support and fear of cancer recurrence in patients with glioma

目的探讨心理弹性在胶质瘤患者领悟社会支持与癌症复发恐惧间的中介作用,为降低患者的复发恐惧提供理论依据。方法采用心理弹性量表(connor-davidson resilience scale,CD-RISC)、领悟社会支持量表(perceived social support scale,PSSS)和癌症患者恐惧疾病进展简化量表(fear of progression questionnaire-short form,FoP-Q-SF)对128例胶质瘤患者进行横断面调查。采用结构方程模型分析心理弹性在胶质瘤患者领悟社会支持与癌症复发恐惧间的中介效应。结果胶质瘤患者癌症复发恐惧得分为(29.52±8.30)分,其中47例患者出现复发恐惧心理功能失调(总分≥34分),发生率为36.7%。癌症复发恐惧、心理弹性、领悟社会支持三者存在相关性(P<0.01)。心理弹性在胶质瘤患者领悟社会支持与癌症复发恐惧间结构方程模型拟合度好,模型较为合理。心理弹性在领悟社会支持与癌症复发恐惧间起中介作用(P<0.05),中介效应为48.4%。结论有36.7%胶质瘤患者存在复发恐惧心理功能失调。可通过提高胶质瘤患者的领悟社会支持来提升心理弹性水平,进而降低患者的癌症复发恐惧水平。

Objective To investigate the mediating effect of resilience on the perceived social support and fear of cancer recurrence in patients with glioma and provide theoretical basis for reducing FCR.Methods A total of 128 patients with glioma ware enrolled in the cross section survey by connor-davidson resilience scale(CD-RISC),Perceived social support scale(PSSS)and Chinese version of fear of progression questionnaire-short form(FoP-Q-SF).Structural equation model was used to investigate the mediating effect of resilience.Results The total score of fears of recurrence was(29.52±8.30).A total of 47 patients had fears of recurrence(total score≥34),with the incidence of 36.7%.There was a correlation among fear of cancer recurrence,resilience and perceived social support(P<0.01).Resilience was a mediator variable between perceived social support and fear of cancer recurrence,with the mediating effect of 48.4%.Conclusion The fear of cancer recurrence among 36.7%glioma patients is malfunltioned,which can be directly reduced by strengthening the perceived social support,or indirectly by improving the level of resilience.
