16种呼吸道病原体的基因芯片法检测儿童呼吸道病原体的价值 被引量:1

Clinical values of a gene chip of 16 respiratory pathogens in the diagnosis of common pathogens in children with respiratory infections

摘要 目的探讨16种呼吸道病原体的基因芯片法对儿童呼吸道病原体的检测价值。方法收集2019年1月至2020年1月洛阳市妇幼保健院收治的85例疑似呼吸道感染患儿的痰液标本,分别采用呼吸道病原体核酸环介导恒温扩增芯片法(LAMP)、病原体分离与培养技术检测病原体。比较患儿呼吸道病原体的检出率及病原体检测符合率。结果85例痰标本中,病原体分离与培养结果显示病原体阳性标本31例,阳性率为36.47%;LAMP检查结果显示病原体阳性标本42例,阳性率为49.41%。病原体分离与培养显示阳性的31例痰标本中,支原体感染15例,占17.65%;铜绿假单胞菌感染8例,占9.41%;耐甲氧西林葡萄球菌感染7例,占8.24%;LAMP检查结果显示阳性的42例痰标本中,支原体感染22例,占25.88%;铜绿假单胞菌感染10例,占11.76%;耐甲氧西林葡萄球菌感染8例,占9.41%。经Kappa一致性分析显示,一致性较高的病原体分别为支原体、铜绿假单胞菌、耐甲氧西林葡萄球菌、鲍曼不动杆菌、肺炎克雷伯杆菌、金黄色葡萄球菌。结论16种呼吸道病原体的基因芯片可快速、高效检测儿童呼吸道病原体,为临床诊断和治疗提供可靠依据。 Objective To investigate the clinical values of a gene chip of 16 respiratory pathogens in the diagnosis of common pathogens in children with respiratory infections.Methods Sputum samples from 85 children with suspected respiratory tract infection who were treated from January 2019 to January 2020 in Luoyang Maternal and Child Health Hospital were collected.The pathogens were tested by loop-mediated isothermal am-plification(LAMP),pathogen isolation and culture technology.Then the detection rate and coincidence rate of pathogens in children’s respiratory tract detected by two methods were compared.Results Positive results of the pathogens were found in 31 samples by pathogen isolation and culture technology,in 42 samples by LAMP,with the positive ratio of 36.47%and 49.41%,respectively.Among the 31 sputum samples with pathogen isolation and culture technology,Mycoplasma infection was found in 15 samples(17.65%),Pseudomonas aeruginosa infection in 8 samples(9.41%)and Methicillin-resistant Staphylococcus infection in 7 samples(8.24%);meanwhile,among the 42 sputum samples with positive LAMP,there were 22 cases(25.88%)of Mycoplasma infection,10 cases(11.76%)of Pseudomonas aeruginosa infection and 8 cases(9.41%)of Methicillin-resistant Staphylococcus infection.Kappa analysis showed that the pathogens with high consistency of two methods were Mycoplasma,Pseudomonas aeruginosa,Methicillin-resistant Staphylococcus,Acinetobacter baumannii,Klebsiella pneumoniae and Staphylococcus aureus.Conclusions Application of gene chip of 16 pathogens can rapidly and effectively detect the pathogens in children with lower respiratory tract infections,which can provide reliable basis for clinical diagnosis and treatment.
