活血祛瘀汤治疗骨折合并软组织挫伤临床研究

Clinical Study on Huoxue Quyu Tang for Fracture Complicated with Soft Tissue Contusion

目的:观察活血祛瘀汤治疗骨折合并软组织挫伤的临床疗效。方法:将76例骨折合并软组织挫伤手术治疗患者随机分为观察组和对照组,每组38例。对照组术后给予常规治疗,观察组在对照组的基础上加用活血祛瘀汤治疗。5 d为1个疗程,治疗2个疗程后比较2组疼痛、肿胀改善程度,治疗前后血液流变学指标变化,骨折愈合时间及骨痂密度变化。结果:治疗后,肿胀Ⅰ度患者观察组占63.16%,对照组占34.21%,2组比较,差异有统计学意义(P<0.05)。术后1 d,2组全血黏度、红细胞压积、纤维蛋白原水平及视觉模拟评分法(VAS)评分比较,差异均无统计学意义(P>0.05);治疗后,观察组上述各项指标均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组患者骨折愈合时间短于对照组,骨痂密度高于对照组,差异均有统计学意义(P<0.05)。结论:活血祛瘀汤可改善骨折合并软组织挫伤患者血液流变学指标,促进骨折愈合,消肿止痛疗效显著。

Objective: To observe the clinical effect of Huoxue Quyu tang for fracture complicated with soft tissue contusion. Methods: A total of 76 cases of patients with fracture complicated with soft tissue contusion and undergoing operation were randomly divided into the observation group and the control group,38 cases in each group. After operation,the control group was given routine treatment,and the observation group was additionally treated with Huoxue Quyu tang based on the treatment of the control group. Both groups were treated for two courses, five days being a course. After treatment,the improvement of pain and swelling,the changes in hemorheological indexes before and after treatment,the healing time of fracture and the callus density were compared between the two groups. Results: After treatment, the percentage of patients with Ⅰ-degree swelling was 63.16% in the observation group and 34.21% in the control group,the difference being significant(P<0.05). On the first day after operation,when compared the levels of whole blood viscosity,hematocrit and fibrinogen as well as the scores of VAS between the two groups,there was no significance in differences(P>0.05);after treatment, the above indexes in the observation group were all lower than those in the control group,differences being significant(P<0.05). After treatment,the healing time of fracture in the observation group was shorter than that in the control group,and the callus density was higher than that in the control group,differences being significant(P<0.05). Conclusion:For patients with fracture complicated with soft tissue contusion,Huoxue Quyu tang can improve their hemorheological indexes,and promote fracture healing,with significant curative effect of detumescence and analgesia.
