刺络放血疗法联合布洛芬内服及中药外敷治疗梨状肌综合征临床研究

Clinical Study on Blood-Letting Puncture Therapy Combined with Oral Administration of Ibuprofen and External Application with Chinese Herbal Medicine for Piriformis Muscle Syndrome

目的:观察刺络放血疗法联合布洛芬内服及中药外敷治疗梨状肌综合征(PMS)的临床疗效。方法:将PMS患者114例随机分为对照组57例与观察组57例,对照组给予布洛芬缓释胶囊口服联合中药外敷治疗,观察组在对照组的基础上行刺络放血疗法。观察2组临床疗效及不良反应,治疗前后疼痛视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分和胫神经H反射波幅、潜伏期。结果:总有效率观察组为94.74%,对照组为80.70%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组患者ODI、VAS评分均较治疗前明显降低(P<0.05),且观察组ODI、VAS评分明显低于对照组(P<0.05)。治疗后,2组患者胫神经H反射潜伏期均较治疗前缩小(P<0.05),且观察组明显小于对照组(P<0.05);2组患者胫神经H反射波幅治疗前后变化不大(P>0.05)。治疗期间2组患者均未发生局部或全身不良反应情况。结论:刺络放血疗法联合布洛芬内服、中药外敷治疗PMS,可显著改善患者临床症状,提高其日常活动能力,临床疗效显著。

Objective:To observe the clinical effect of blood-letting puncture therapy combined with oral administration of ibuprofen and external application with Chinese herbal medicine for piriformis muscle syndrome(PMS). Methods:A total of114 cases of PMS patients were randomly divided into the control group and the observation group,57 cases in each group.The control group was given oral administration of ibuprofen sustained release capsules combined with external application with Chinese herbal medicine,and the observation group was additionally given blood-letting puncture therapy based on the treatment of the control group. The scores of Visual Analogue Scale(VAS) and Oswestry Disability Index(ODI), and the amplitude and latency of H-reflex of tibial nerve before and after treatment as well as the clinical effect and adverse reactions in the two groups were observed. Results:The total effective rate was 94.74% in the observation group and 80.70% in the control group,the difference being significant(P<0.05). After treatment,the scores of ODI and VAS in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the scores in the observation group were significantly lower than those in the control group(P<0.05). After treatment,the latency of H-reflex of tibial nerve in the two groups was shortened when compared with that before treatment(P<0.05), and the one in the observation group was shorter than that in the control group(P<0.05);before and after treatment,there was no obvious change in the comparison of amplitude of H-reflex of tibial nerve between the two groups(P>0.05). There was no local or general adverse reaction in the two groups during treatment. Conclusion: The therapy of blood-letting puncture therapy combined with oral administration of ibuprofen and external application with Chinese herbal medicine for PMS can significantly improve clinical symptoms and activity of daily living with significant clinical effect.
