彩色多普勒超声诊断肾上腺囊性占位性病变18例报告

Color doppler ultrasound in diagnosis of adrenal cystic tumor(18 cases report)

目的:探讨肾上腺囊性占位性病变的超声表现,以提高诊断水平。方法:回顾性分析18例经手术病理证实的肾上腺囊性占位性病变的超声特征,其中囊肿14例,囊性畸胎瘤2例,嗜铬细胞瘤1例,腺瘤1例。结果:肾上腺囊肿表现为无回声区,合并钙化者边缘可见强回声环,合并感染者声像图表现多样;肾上腺囊性畸胎瘤表现为囊性为主的混合回声肿块,无血流信号;嗜铬细胞瘤和腺瘤均表现为厚壁囊性肿物。结论:肾上腺囊性占位性病变有一定的超声特征,大部分肾上腺囊肿具有较明确的超声特征,复杂的囊肿、囊性畸胎瘤、囊性变的嗜铬细胞瘤及腺瘤诊断较困难。

Objective:To evaluate the sonographic findings of adrenal cystic lesions,in order to improve the diagnosis level.Methods:The sonographic features of 18 adrenal cystic lesions,which were confirmed by surgical pathology,were analyzed retrospectively,including 14 cases of cyst,2 cases of cystic teratoma,pheochromocytoma in 1 case,adenoma in 1 case.Results:The sonographic appearance of adrenal cysts were anechoic,some with calcification ring,varied ultrasonographic performance in infected cyst.Adrenal cystic teratomas characterized by mixed echo lumps without blood flow signals.Pheochromocytoma and adenomas are characterized by cystic neoplasm with thick wall.Conclusion:Adrenal cystic lesions has certain sonographic features.Most of adrenal cyst has a specific ultrasonic characteristics.The diagnosis of complicated cyst,cystic teratoma,cystic pheochromocytoma and adenomas were difficult.
