呼吸道感染并发心肌炎患者血清MCP-1与 NF-κB及CypA/CD_(147)的表达

Changes of serum MCP-1,NF-κB and CypA/CD147 in patients with myocarditis complicated with respiratory tract infection

目的探究呼吸道感染并发心肌炎患者血清降钙素原(PCT)、单核细胞趋化蛋白1(MCP-1)、核因子kB(NF-κB)及亲环素A(CypA)/细胞外基质金属蛋白酶诱导因子(CD_(147))的水平表达。方法选取2016年8月-2018年3月河南省人民医院86例呼吸道感染诱发心肌炎患者作为病例组,86名同期体检的健康人为对照组。检测两组血清PCT、MCP-1、NF-κB、CypA/CD_(147)水平,比较不同患病程度下CypA、CD_(147)的表达情况;评估血清PCT、MCP-1、NF-κB及CypA、CD_(147)的表达水平在呼吸道感染诱发心肌炎诊断中的诊断效能。结果与对照组比,病例组患者血清PCT、MCP-1、NF-κB、CypA、CD_(147)水平高于对照组,差异具有统计学意义(P<0.05);CypA、CD_(147)在重度患者中的表达水平高于轻度与中度患者,在中度患者的表达水平高于轻度患者,差异有统计学意义(P<0.05)。血清PCT、MCP-1、NF-κB、CypA、CD_(147)对呼吸道感染诱发心肌炎患者具有一定的诊断价值;且联合检测CypA与CD_(147)的灵敏度、特异性高于单一指标诊断,差异有统计学意义(P<0.05)。结论呼吸道感染诱发心肌炎患者血清PCT、MCP-1、NF-κB水平均升高,CypA与CD_(147)联合诊断呼吸道感染诱发心肌炎的临床价值较高。

OBJECTIVE To explore the expression of serum procalcitonin(PCT),monocyte chemoattractant protein 1(MCP-1),nuclear factorκB(NF-κB),cyclophilin A(CypA)/extracellular matrix metalloproteinase inducer(CD147)in the patients with myocarditis induced by respiratory tract infection.METHODS Totally 86 patients with myocarditis induced by respiratory tract infection who were treated in Henan Provincial People’s Hospital from Aug 2016 to Mar 2018 were chosen as the case group,meanwhile,86 healthy people who received physical examination were set as the control group.The levels of serum PCT,MCP-1,NF-κB and CypA/CD147 of the two groups were detected,the expression of CypA and CD147 was compared among the patients with different severities,and the efficiencies of the serum PCT,MCP-1,NF-κB,CypA and CD147 in diagnosis of myocarditis induced by respiratory tract infection were evaluated.RESULTS The levels of serum PCT,MCP-1,NF-κB,CypA and CD147 of the case group were significantly higher than those of the control group(P<0.05);the levels of CypA and CD147 of the severe patients were significantly higher than those of the mild and moderate patients,the levels of above indexes of the moderate patients were significantly higher than those of the mild patients(P<0.05).The serum PCT,MCP-1,NF-κB,CypA and CD147 showed certain value in diagnosis of the patients with myocarditis induced by respiratory tract infection;the sensitivity and specificity of the joint detection with CypA and CD147 were significantly higher than those of the single index(P<0.05).CONCLUSION The levels of serum PCT,MCP-1 and NF-κB of the patients with myocarditis induced by respiratory tract infection are elevated,and the joint detection with CypA and CD147 has high clinical value in diagnosis of myocarditis induced by respiratory tract infection.
