摘要
目的探讨衣原体引起的盆腔炎与衣原体热休克蛋白60(CHSP60)抗体和人类白细胞抗原DQ(HLA-DQ)基因的相关性,为盆腔炎发病机制提供依据。方法选择2017年1月-2019年1月海南省妇幼保健院收治的沙眼衣原体感染盆腔炎患者142例作为盆腔炎组,再随机抽取医院健康体检的健康妇女103名作为对照组,检测并比较两组受试者CHSP60抗体表达情况及HLA-DQA、B等位基因分型分布。结果盆腔炎组患者CHSP60抗体阳性率、HLA-DQA*0501以及HLA-DQB*0301等位基因阳性率均高于对照组(P<0.05),而其余HLA-DQA、B等位基因分布上两组无统计学差异。在盆腔炎患者中,HLA-DQA*0501以及HLA-DQB*0301等位基因阳性患者CHSP60抗体阳性率高于HLA-DQA*0501以及HLA-DQB*0301等位基因阴性患者(P<0.05)。结论 HLA-DQA*0501以及HLA-DQB*0301可能是衣原体引起盆腔炎发生的易感基因,且盆腔炎患者HLA-DQA*0501以及HLA-DQB*0301与CHSP60抗体表达密切相关,提示衣原体引起盆腔炎免疫反应受遗传因素的调控。
OBJECTIVE To explore the correlation between Chlamydia-induced pelvic inflammation and Chlamydia heat shock protein 60 antibody and human leukocyte antigen DQ(HLA-DQ)gene so as to provide evidence for pathogenesis of pelvic inflammation.METHODS From Jan 2017 to Jan 2019,totally 142 patients with Chlamydia trachomatis pelvic inflammation who were treated in Women and Children Hospital of Hainan Province were assigned as the pelvic inflammation group,and 103 healthy women who received physical examination were randomly selected as the control group.The expression of CHSP60 antibody and genotypes of HLA-DQA and B alleles were detected and compared between the two groups of subjects.RESULTS The positive rates of CHSP60 antibody,HLA-DQA*0501 and HLA-DQB*0301 alleles of the pelvic inflammation group significantly were higher than those of the control group(P<0.05),however,there was no significant difference in the distribution of other HLA-DQA,B alleles.Among the patients with pelvic inflammation,the positive rate of CHSP60 antibody of the patients who were positive for HLA-DQA*0501 and HLA-DQB*0301 alleles was significantly higher than that of the patients who were negative for HLA-DQA*0501 and HLA-DQB*0301 alleles(P<0.05).CONCLUSION The HLA-DQA*0501 and HLA-DQB*0301 may be the susceptible genes to Chlamydia-induced pelvic inflammation,and the HLA-DQA*0501 and HLA-DQB*0301 are closely related to the expression of CHSP60 antibodies in the patients with pelvic inflammation,indicating that the immune response of Chlamydia-induced pelvic inflammation is regulated by genetic factors.
