不同长度PFNA内固定治疗老年股骨转子间骨折的临床效果比较

Comparison of clinical effect of different length PFNA internal fixation in the treatment of intertrochanteric fracture in the elderly

目的比较不同长度股骨近端髓内钉(PFNA)内固定治疗老年股骨转子间骨折的临床效果。方法选取2016年1月至2019年3月我科采用不同长度PFNA内固定术治疗的62例老年股骨转子间骨折患者为研究对象,按PFNA主钉长短将其分为短钉组(170 mm)和长钉组(200 mm),各31例。比较两组患者的手术相关指标、输血率、髋关节功能、住院天数、术后并发症发生情况、骨折愈合时间。结果短钉组手术时间短于长钉组,术中失血量及隐性失血量少于长钉组,输血率低于长钉组(P<0.05)。结论短主钉PFNA内固定治疗Ⅰ~Ⅲ型老年股骨转子间骨折可缩短手术时间,减少出血量,降低输血率,利于患者康复。

Objective To compare the clinical effect of different length of proximal femoral nail(PFNA)internal fixation in the treatment of intertrochanteric fracture in the elderly.Methods From January 2016 to March 2019,62 elderly patients with intertrochanteric fracture were treated with PFNA of different lengths in our department were selected as the study objects,and the patients were divided into short nail group(170 mm)and long nail group(200 mm)according to the length of PFNA main nail,with 31 cases in each group.The operation related indexes,blood transfusion rate,hip joint function,hospitalization days,postoperative complications and healing time of fracture were compared between the two groups.Results The operation time of the short nail group was shorter than that of the long nail group,the intraoperative blood loss and hidden blood loss were less than those of the long nail group,and the blood transfusion rate was lower than that of the long nail group(P<0.05).Conclusion Short main nail PFNA internal fixation in the treatment of typeⅠ-Ⅲintertrochanteric fracture in the elderly can shorten the operation time,reduce the amount of bleeding and the blood transfusion rate,which is conducive to the rehabilitation of patients.
