B超定位髂腹下/髂腹股沟神经阻滞在下腹部开腹手术中的应用效果

Application effect of B-ultrasound positioning ilioabdominal/ilioinguinal nerve block in lower abdominal open surgery

目的探究B超定位髂腹下/髂腹股沟神经阻滞在下腹部开腹手术中的应用效果。方法将2018年1月至2019年5月行下腹部开腹手术的100例患者以单双号编号法随机分为对照组和试验组,每组50例。对照组给予全身麻醉,试验组同时实施B超定位髂腹下/髂腹股沟神经阻滞。比较两组麻醉效果。结果试验组麻醉优良率为100.00%,高于对照组的86.00%(P<0.05)。试验组手术用时、手术后即刻疼痛程度、术后活动时间、术后30 h内舒芬太尼用量、术后4 h舒适度评分均优于对照组(P<0.05)。试验组不良反应总发生率为12.00%,低于对照组的32.00%(P<0.05)。结论B超定位髂腹下/髂腹股沟神经阻滞应用于下腹部开腹手术患者中,能够获得更好的麻醉效果,对患者生理功能造成的影响较小,且安全性高,值得临床推广应用。

Objective To explore the application effect of B-ultrasound positioning ilioabdominal/ilioinguinal nerve block in lower abdominal open surgery.Methods A total of 100 patients undergoing lower abdominal open surgery from January 2018 to May 2019 were randomly divided into control group and experimental group by the method of single and even numbers numbering,with 50 cases in each group.The control group was given general anesthesia,while the experimental group was given B-ultrasound positioning ilioabdominal/ilioinguinal nerve block at the same time.The anesthetic effects of the two groups were compared.Results The excellent and good rate of anesthesia in the experimental group was 100.00%,which was higher than 86.00%in the control group(P<0.05).The operation time,postoperative immediatepain degree,postoperative activity time,sufentanil dosage within 30 h after operation and comfort score at 4 h after operation in the experimental group were better than those in the control group(P<0.05).The total incidence of adverse reactions in the experimental group was 12.00%,which was lower than 32.00%in the control group(P<0.05).Conclusion B-ultrasound positioning ilioabdominal/ilioinguinal nerve block applied to lower abdominal open surgery patients can obtain better anesthesia effect,it has less impact on the physiological function of patients and higher safety,which is worthy of clinical promotion and application.
