董耀荣治疗扩张性心肌病经验

DONG Yaorong’s Experience in Treatment of Dilated Cardiomyopathy

介绍董耀荣教授治疗扩张性心肌病的临床经验。扩张性心肌病是指患者心肌无原因增大,左或右或双心室功能受损导致心脏进一步肥大的心肌疾病。董师认为本病的病机特点是心肾阳气虚衰为本,水饮瘀血为标,属本虚标实之证,病程中和虚、毒、痰、瘀四者关系密切,治疗提倡无论早中晚期,养血活血应该贯穿始终,需重视心肾同调,强调补气温阳同时不能忽视养阴,治以补气温阳,化瘀利水,滋阴益肾。董师治疗本病经验丰富,采用自拟强心方加减对治疗扩张性心肌病疗效突出,强心方由五苓散、真武汤加减化裁而成,全方配伍体现了董师养血活血贯穿始终,重视心肾同调的学术思想,有恢复心脏结构的效果,长期服用本方可以明显稳定病情。特举验案1则说明疗效思路。

This article introduced Professor DONG Yaorong’s clinical experience in treatment of dilated cardiomyopathy.Dilated cardiomyopathy(DCM) is a kind of myocardial disease in which the heart is enlarged without cause,and the function of left or right or double ventricles is damaged,leading to further hypertrophy of the heart.According to DONG,the pathogenesis of the disease is characterized by the deficiency of heart and kidney Yang and the failure of Qi,which is based on fluid retention and blood stasis.It is a syndrome of deficiency in root and excess in treetop.During the course of the disease,there is a close relationship among deficiency,toxin,phlegm and blood stasis.No matter in the early,middle and late stages of the treatment,nourishing blood and promoting blood circulation should be throughout the whole process.Attention should be paid to the coordination of heart and kidney,strengthening the regulation of Qi and Yang,and at the same time,nourishing Yin should not be ignored,so as to nourish Qi and Yang,remove blood stasis and promote urination,nourish Yin and benefit kidney.DONG has rich experience in treatment of this disease,and Qiangxin Formula has outstanding effect in treatment of dilated cardiomyopathy.Qiangxin Formula is composed of Wuling Powder and Zhenwu Decoction.The compatibility of the whole formula reflects DONG’s academic thought of nourishing blood and promoting blood circulation throughout the whole process,paying attention to the harmony of heart and kidney,which has the effect of restoring the heart structure.Taking this formula for a long time can significantly stabilize the condition.One case of clinical trial was cited to illustrate the idea of curative effect.
