摘要
目的:探讨优化术前禁饮食对颈椎骨折并脊髓损伤前路手术患者快速康复的影响。方法:选取我院脊柱外科2018年1月-2019年1月颈椎骨折并脊髓损伤前路手术患者84例,依照随机数字表法分为观察组40例,对照组44例进行研究。观察组采用术前禁饮食优化流程,依据患者手术时间,术前给予气管推移试验、分时段给予麦芽糊精。对照组采用术前8小时禁食6小时禁饮,术前给予气管推移试验方案。比较2组患者术前禁饮食时间、主观感受(口渴、胃部不适、饥饿、乏力)、血糖情况,同时比较2组患者住院时间。结果:观察组患者的术前禁饮时间短于对照组,差异有统计学意义(P<0.01);观察组患者的术前禁食时间短于对照组,差异有统计学意义(P<0.01)。主观感受方面,观察组患者术前焦虑、口渴、胃部不适、饥饿、乏力评分低于对照组,差异具有统计学意义(P<0.05)。观察组患者术前1小时血糖高于对照组,差异有统计学意义(P<0.05)。观察组患者住院时间短于对照组,差异有统计学意义(P<0.05)。结论:优化术前禁饮食流程缩短了术前禁饮食的时间,加速患者康复,安全有效。
Objective:To explore the effect of preoperative banned diet on the rapid recovery of patients with cervical spine fractures and anterior spinal cord injury.Methods:Eighty-six patients with cervical spine fractures and anterior spinal cord injury from January 20 to January 1919 in our hoptal were enolled.Acording to the random number table method,40 patients were divided into the experimen-tal group and 44 patients in the control group.The experimental group used the preoperative banned diet optimization process.According to the patients operation time,the tracheal shift test was given before the operation,and the maltodextrin was given in a time interval.The control group was fasted for 6 hours before surgery for 6 hours,and the tracheal shift test protocol was given before surgery.The preopera-tive banned diet time,subjctive feelings(thirst,stomach discomnfort,hunger,fatigue)and blood glucose were compared between the two groups,and the hospital stay Was compared between the two groups.Results:The preoperative fasting time of the patients in the experi-mental group was shorter than that in the control group,and the difference was statistically significant(P<0.01).The preoperative fast-ing time of the experimental group W88 shorter than that of the control'group,and the difference was statistically significant(P<0.01).Subjective feelings,the preoperative anxiety,thirst,stomach discomfort,hunger and fatigue scores of the experimental group were lower than the control group,the difference was statistically significant(P<0.05).The blood glucose level in the experimental group was high-er than that in the control group 1 hour before operation,and the difference was statistically significant(P<0.05).The hospitalization time of the experimental group was shorter than that of the control group,and the difference was statistically significant(P<0.05).Con-clusion:Optimizing the pre-operative banned diet couldshorten the time for pre-operative banned diet and accelerates patient recovery,safe and effective.
