基于项目反应理论的冠心病自我管理量表项目分析

Analysis of the Coronary Artery Disease Self-Management Scale Based on Item Response Theory

目的应用项目反应理论对冠心病自我管理量表(coronary artery disease self-management scale,CSMS)进行项目分析。方法便利抽样法选取西安市某社区冠状动脉粥样硬化性心脏病(简称冠心病)患者为研究对象,采用CSMS量表对其进行调查。采用项目反应理论计算该量表各条目的区分度,难度系数及项目信息量来评价条目。结果经分析CSMS量表满足单维性假设,可以应用项目反应理论进行条目分析。除条目2,3外,其余各条目区分度系数均在合理取值范围(0.5,3)内。15个条目的难度系数均在合理取值范围(-3.4)内。各条目的最大信息量在0.016~1.495之间,本研究中量表的最大信息量为10.741.对应潜在能力参数值为-0.2。结论CSMS量表的大部分条目区分度尚可,能够有效区分和鉴别不同能力的冠心病患者;难度参数存在劣势,在一定程度上导致了信息量损失。CSMS量表能够为自我管理能力水平中等及偏低的冠心病患者提供更为精准的评估。

Objective To conduct the item analysis of the coronary artery disease self-management scale(CSMS) based on item response theory(IRT).Methods Data was collected from the survey on self-management behavior regarding coronary artery disease of residents who lived in the Xi’an community.CSMS was used to investigate.Using item response theory to calculate the discrimination, difficulty coefficient and item information of each item in the scale.Results After the analysis,CSMS satisfied the unidimensional assumption,and the IRT could be used for item analysis.Except for item 2 and item 3,all items held the rational discrimination parameters(0.5,3);and 15 items held the rational difficulty parameters(-3,4).The number of information provided by each item ranged from 0.016 to 1.495.In this study,the maximum information of the test was 10.741,and the corresponding potential ability parameter value was-0.2.Conclusions Most of the items in the questionnaire with acceptable discriminations could differentiate residents at a different level of competence.The disadvantage of difficulty parameters leads to the loss of information.CSMS provided a more accurate assessment of residents with the medium and low-level ability of coronary heart disease self-management.
