佩戴角膜塑形镜对正负相对调节力的影响

Influence of wearing corneal orthopedic lens on the positive and negative relative accommodation

目的观察佩戴角膜塑形镜对正负相对调节力的影响。方法前瞻性病例对照研究。本研究共纳入200例自2013年1月至2014年6月濮阳市眼科医院就诊的青少年近视,随机分为两组,佩戴角膜塑形镜者作为试验组,未佩戴角膜塑形镜者作为对照组,每组100例。对比两组间正负相对调节力和试验组佩戴前后正负相对调节力。结果佩戴2个月后,试验组负相对调节力与对照组差异无统计学意义(t=0.955,P=0.340),而正相对调节力则优于对照组,两组差异有统计学意义(t=11.225,P=0.000)。试验组佩戴后正相对调节力优于佩戴前(t=9.763,P=0.000),而负相对调节力则佩戴前后差异无统计学意义(t=0.994,P=0.321)。结论正相对调节力是延缓近视进展的重要机制,佩戴角膜塑形镜在青少年近视者中具有良好的临床效果,能够有效提高正相对调节力。

Objective To observe the Influence of wearing corneal orthopedic lens(orthokerotologic lens)on the positive and negative relative accommodation.Methods This was a prospective case-control study.In this study,200 cases of juvenile myopia in the Ophthalmologic Hospital of Puyang from Jan.2013 to Jun.2014 were randomly divided into two groups,100 cases in each group.The patients wore orthokeratologic lens were included in trial group and the others were included in control group without orthokeratologic lens.The positive and negative relative accomodation between the two groups was compared,which was compared before and after wearing orthokeratology lens in the trial group.Results At 2 months after wearing,the difference of negative relative accommodation was not statistically significant between the two groups(t=0.955,P=0.340).But after wearing,positive relative accommodation of trial group was better than that of control group(t=11.225,P=0.000).In the trial group,positive relative accommodation after wearing orthokeratologic lens was better than that before wearing(t=9.763,P=0.000),and there was no significant difference in negative relative accommodation(t=0.994,P=0.321).Conclusion Positive relative accomodation is an important mechanism to delay the progress of myopia.After wearing corneal orthopedic lens,there is good clinical efficacy in juvenile myopia and can effectively improve the positive relative accomodation.
