子宫内膜癌术后行TC化疗方案联合放疗的疗效及对患者FACT-G评分及预后的影响

Efficacy of TC Chemotherapy Regimen combined with Radiotherapy on Postoperation of Endometrial Cancer Surgery and Its Effects on FACT-G Score

目的观察TC化疗方案联合放疗治疗子宫内膜癌(EC)术后的疗效及对患者癌症患者生命质量测定量表(FACT-G)评分、复发率和3年生存率的影响。方法回顾性分析76例EC患者临床资料,将术后行TC方案化疗联合放疗的42例患者纳入联合组,余34例术后仅行放疗的患者纳入对照组,比较2组患者入组时及随访3个月时2组患者FACT-G评分差异,记录不良反应情况,观察复发、转移情况,统计3年生存率。结果随访3个月时,2组患者FACT-G各方面评分均较入组时降低,且联合组低于对照组(P<0.05)。不良反应除骨髓抑制发生率联合组高于对照组外(P<0.05),其余2组间无显著性差异(P>0.05)。复发、远处转移率联合组低于对照组(P<0.05)。经Log-Rank分析,联合组3年生存率92.86%高于对照组76.47%(P<0.05)。结论相较于单纯放疗,EC术后予以TC化疗方案联合放疗治疗有助于改善患者生存质量,改善复发、转移及远期生存情况,有一定优势,但应加强对联合治疗骨髓抑制发生情况的关注。

Objective To observe the efficacy of TC chemotherapy regimen combined with radiotherapy on postoperation of endometrial cancer(EC)surgery and its effects on score of quality of life-patients with cancer(FACT-G),recurrence rate and 3-year survival rate.Methods The clinical data of 76 patients with EC were retrospectively analyzed.42 patients who underwent TC regimen chemotherapy combined with radiotherapy were included in combined group,and the remaining 34 patients who underwent radiotherapy only were included in the control group.The FACT-G scores were compared between the 2 groups at entering the group and at 3 months of follow-up.The adverse reactions were recorded,and the recurrence and metastasis were observed,and the 3-year survival rate was calculated.Results At 3 months follow-up,the scores of all aspects of FACT-G in the 2 groups were lower than those compared with those before entering the group,and the scores in combined group were lower than those in the control group(P<0.05).The incidence rate of myelosuppression in combined group was higher than that of the control group(P<0.05).There were no significant differences in the other adverse reactions between the 2 groups(P>0.05).The recurrence rate and distant metastasis rate in combined group were lower than those of the control group(P<0.05).The 3-year survival rates in combined group and the control group were 92.86%(39/42)and 76.47%(26/34)respectively.Log-Rank analysis showed that combined group was higher than that of the control group(χ^2=4.459,P<0.05).Conclusion TC chemotherapy regimen combined with radiotherapy after EC can improve the quality of life,reduce the recurrence rate and distant metastasis rate,and improve the 3-year survival rate.It has certain advantages over radiotherapy alone,but the combined treatment has a higher incidence rate of myelosuppression,which should be strengthened.
