摘要
目的探讨超声骨刀微创拔牙法与传统拔牙法拔除下颌水平阻生智牙的临床效果。方法选取2017年8月至2019年8月本所收治的下颌水平阻生智牙摘除患者300例作为研究对象,依据拔牙方式分为对照组(n=150,采用传统拔牙法)和实验组(n=150,采用经超声骨刀下微创拔牙法),对比两组手术时间、张口受限度、并发症、疼痛状况和生活质量。结果实验组手术时间明显短于对照组(P<0.05);实验组邻牙损伤、神经损伤与骨组织损伤等并发症发生率明显低于对照组(P<0.05);实验组张口受限度与术后疼痛状况改善效果优于对照组(P<0.05);实验组躯体功能、社会能力、物质水平、心理功能及总活力评分明显高于对照组(P<0.05)。结论采取经超声骨刀微创拔牙法拔除下颌水平阻生智牙效果显著,可减少手术并发症,缩短手术时间,减轻躯体疼痛,同时,还能促进术后生活质量提升,值得临床推广运用。
Objective To explore the clinical effect of minimally invasive extraction with ultrasonic bone knife and traditional extraction method on impacted wisdom teeth at the lower jaw level.Methods From August 2017 to August 2019,300 cases of mandibular level impacted wisdom tooth extraction patients treated at our institution were selected as the research subjects,and they were divided into a control group(n=150,using traditional tooth extraction method)and an experimental group(n=150,using a minimally invasive tooth extraction method with an ultrasonic bone knife)according to the method of tooth extraction,compared the operation time,opening restriction,complications,pain status and quality of life were compared between two groups.Results The operation time of the experimental group was significantly shorter than that of the control group(P<0.05).The incidence of complications such as adjacent tooth injury,nerve injury and bone tissue injury in the experimental group was significantly lower than that of the control group(P<0.05).The postoperative pain improvement effect was better than the control group(P<0.05).The physical function,social ability,material level,mental function and total vitality score of the experimental group were significantly higher than the control group(P<0.05).Conclusion The minimally invasive tooth extraction with ultrasonic bone knife has a significant effect on the extraction of impacted wisdom teeth at the level of the mandible.It can reduce surgical complications,shorten the operation time,reduce physical pain,and at the same time promote the quality of life after surgery.
