多项尿检对系统性红斑狼疮肾损害诊断的临床价值分析

Analysis of the clinical value of multiple urine tests in the diagnosis of renal damage in systemic lupus erythematosus

目的探讨分析多项尿检对系统性红斑狼疮(SLE)肾损害诊断的临床价值。方法选取90例SLE患者作为研究组,另选取同期90例进行体检健康人群作为对照组。两组进行尿蛋白定性以及尿N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、α1-微球蛋白(α1-MG)、微量白蛋白(mALB)水平检测并进行比较。结果经SLE尿蛋白定性检测,对照组均呈阴性,研究组38例患者呈阴性,52例患者呈阳性。研究组SLE尿蛋白阴性患者NAG为(28.00±10.2)U/L、α1-MG为(42.89±24.63)mg/L、mALB为(16.54±10.41)mg/L,阳性患者NAG为(87.92±37.30)U/L、α1-MG为(125.93±73.62)mg/L、mALB为(357.61±132.73)mg/L,对照组受试者的NAG为(12.61±4.44)U/L、α1-MG为(9.34±3.42)mg/L、mALB为(9.3±3.15)mg/。研究组患者NAG、α1-MG、mALB水平均高于对照组,且研究组SLE尿蛋白阳性患者高于阴性,差异具有统计学意义(P<0.05)。结论给予SLE疮肾损害患者尿蛋白定性以及尿NAG、α1-MG、mALB指标等指标检查有利于判断患者肾损伤程度,可作为狼疮肾损害的判断指标,具有临床推广价值。

Objective To discuss and analyze the clinical value of multiple urine tests in the diagnosis of renal damage in systemic lupus erythematosus(SLE).Methods There were 90 SLE patients selected as research subjects,and concurrent 90 healthy persons for physical examination as the control group.The qualitative test of urine protein and the levels of N-acetyl-β-d-glucosaminidase(NAG),α1-microglobulin(α1-mg)and microalbumin(mAlb)were detected and compared between the two groups.Results After qualitative test of SLE urine protein,the control group was all negative,38 patients in the research group were negative,and 52 patients were positive.In the research group,NAG,α1-MG and mALB in patients with negative SLE urine protein were(28.00±10.2)U/L,(42.89±24.63)mg/L,(16.54±10.41)mg/L,and positive patients were(87.92±37.30)U/L,(125.93±73.62)mg/L,(357.61±132.73)mg/L,the control group were(12.61±4.44)U/L,(9.34±3.42)mg/L,(9.3±3.15)mg/L.NAG,α1-MG and mALB of the research group was higher than that of the control group,and patients with positive SLE urine protein was higher than that of negative patients,and the difference was statistically significant(P<0.05).Conclusion Qualitative test of urinary protein,urinary NAG,α1-MG and urinary mALB indexes in SLE patients with renal damage is helpful for judging the degree of renal damage in patients,and it can be used as a judging indicator for lupus renal damage.It contains clinical promotion value.
