硅油填充术后早期硅油进入前房的临床分析及处理

Analysis and treatment of the clinical characteristics of early silicone oil entering the anterior chamber after silicone oil filling

目的观察硅油填充术后早期发生前房进入硅油的影响因素及处理方法。方法回顾性分析安徽省立医院2015年7月至2019年11月进行硅油填充术后早期发生前房硅油的22例(22只眼)患者的临床资料,包括年龄、性别、临床诊断、屈光状态、晶状体情况、术前术后眼压及治疗方法。结果22例患者硅油填充术后早期出现硅油进入前房后13例出现继发性青光眼,但22例患者均行前房硅油取出。手术后观察眼底视网膜复位良好,眼压得到控制,未再发生硅油进入前房。结论复发性视网膜脱离、合并外伤、无晶状体眼、高度近视患者行硅油填充术后相对易发生早期硅油进入前房,硅油进入前房后诱发眼压升高,经前房硅油取出后可较好控制眼压。

Objective To observe the predisposing factors,harms and treatment methods of anterior chamber silicone oil after silicone oil filling.Methods Retrospective analysis of the clinical data of 22 patients with anterior chamber silicone oil during the early period after silicone oil filling from July 2015 to December 2019 in Anhui Provincial Hospital,including age,ender,clinical diagnosis,refractive status,crystal condition,intraocular pressure before and after operation,and treation.Results Thirteen in twenty-two patients had secondary glaucoma after the silicone oil entered the anterior chamber early after silicone oil filling,but all 22 patients had the anterior chamber silicone oil removed.The anterior chamber silicone oil can be removed by using saline replacement or viscoelastic replacement silicone oil.After the operation,the fundus and retina are well reset,the intraocular pressure is controlled,and the silicone oil does not enter the anterior chamber again.Conclusion Patients with recurrent retinal detachment,combined trauma,aphakia,and high myopia are relatively prone to early silicone oil entering the anterior chamber after silicone oil filling.Silicone oil enters the anterior chamber and then induces increased intraocular pressure.Silicone oil removing can give good control of intraocular pressure.
