摘要
目的探讨磁共振磁敏感加权成像(SWI)技术对脑微出血(cerebral microbleeds,CMBs)的诊断价值。方法采用回顾性方式分析46例CMBs患者完整资料,所有患者均进行常规序列(T1WI、T2WI、T2Flair、DWI)及SWI检查,观察其影像结果,统计微出血灶的数量、病灶所处位置,比较常规序列与SWI序列对微出血灶检出率的差异,分析SWI序列对CMBs的诊断意义。结果46例患者,共检出微出血灶211处,大小2~8 mm,皮层及皮层下白质区107处,占50.71%;基底节区69处,占32.70%;丘脑23处,占10.90%;小脑7处,占3.31%;脑干5处,占2.37%。微出血灶主要集中在皮层及皮层下白质区以及基底节区。SWI、T1WI、T2WI、T2FLAIR及DWI分别检出的微出血灶数目211处、31处、37处、43处、101处,检出率分别是100%、14.69%、17.54%、20.38%、47.87%,差异有统计学意义(P<0.05)。结论SWI序列较常规序列对微出血灶具有更高的敏感性及检出率,能更清晰直观地显示出病灶,对于临床及时发现治疗,减少相关并发症、提高患者的生活质量具有重要的临床意义。
Objective To evaluate the diagnostic value of magnetic resonance susceptibility weighted imaging(SWI)sequence for cerebral micro-bleedings(CMBs).Methods The complete data of 46 patients with CMBs were analyzed retrospectively.All patients were examined by routine sequence(T1WI,T2WI,T2Flair,DWI)and SWI.Their imaging results were observed,the number and location of micro-bleedings foci were counted,the differences in the detection rate of micro-bleedings foci were compared between conventional sequence and SWI sequence,and the diagnostic significance of SWI sequence for CMBs was analyzed.Results Among the 46 patients,211 micro-bleeding foci were detected,with 2~8 mm in size,among which 107 foci in cortical and subcortical white matter areas were detected,accounting for 50.71%;69 foci in basal ganglia,accounting for 32.70%;23 foci in thalamus,accounting for 10.90%;7 foci in cerebellum,accounting for 3.31%;5 foci in brain stem,accounting for 2.37%.The micro-bleeding foci were mainly in cortical and subcortical white matter area and basal ganglia area.The number of micro-bleeding foci detected by SWI,T1WI,T2WI,T2FLAIR and DWI was 211,31,37,43 and 101 respectively,the detection rates were 100%,14.69%,17.54%,20.38%and 47.87%respectively,and the difference was statistically significant(P<0.05).Conclusion Compared with the conventional sequence,SWI sequence has higher sensitivity and detection rate for micro-bleeding foci,and can show foci more clearly and intuitively.It has important clinical significance for timely clinical discovery and treatment,reduction of related complications,and improvement of patients'quality of life.
