摘要
目的初步分析可穿戴主动健康设备与常规医用检测设备在佩戴者多种生活情境下的数据差异。方法纳入2019年10—12月在中国医学科学院阜外医院健康体检中心体检的18岁以上健康受试者300例,首先进行可穿戴主动健康设备及常规医用检测设备的标准化测量。随后按照随机数字表法将300名受试者随机分为4组,每组75名,分别在电极片接触不良、运动干扰、电磁干扰、导联错误情境下穿戴主动健康设备(快速心电检测仪)及常规医用检测设备(动态心电图仪)各检测1d,测量心率以及心率变异性、动态心电图,并将多种测量方式后的数据进行整合与差异化描述。结果标准化测量结果显示,可穿戴主动健康设备检出的总心搏>10万的比例低于常规医用检测设备,差异有显著性(P<0.01),其他参数的判断方面均差异无显著性(P>0.05)。不同情境下的测量结果与标准化测量结果比较,电极片接触不良时主动健康设备及医用设备的总心搏>10万的检出率均偏低;存在运动干扰时,两种设备早搏及ST段/T波改变的检出率有增高趋势;存在电磁干扰时,两种设备各测量参数未见明显差异化趋势;存在导联错误时,两种设备对心律失常的检出率均未受影响,但ST段/T波异常的检出率有增高趋势。不同情境下可穿戴主动健康设备与常规医用检测设备在各参数的判断方面差异均无显著性(P>0.05)。结论在规范测量及佩戴的前提下,动态心电设备具备较高的精确度及良好的可靠性,有必要普及相关规范,将可穿戴主动健康设备用于主动健康管理,从而提高人群自我健康管理能力,降低心血管事件发生率。
Objective To analyze the data difference between wearable active health equipment and conventional medical testing equipment in various living situations.Method The aim of this study was to compare the data variation between portable medical devices and devices used in medical facilities for measurement of cardiovascular data.A total of 300 healthy participants were enrolled for measurement of 24-hour electrocardiogram.Participants were also randomized into diverse real-life conditions for measurement of possible data bias.Result Under standardized measurement of 24-hour electrocardiogram,there was no significant difference between results of wearable and medical devices.The case detection rates of arrythmias and myocardial ischemia tend to be lower in wearable devices than medical devices when worn in wrong manners.Conclusion Domestic wearable devices for cardiovascular data monitoring are accurate and reliable when adopted correctly.Using wearable devices for active cardiovascular health management is feasible and may help to reduce cardiovascular events.
