乳糜泻抗体联合基因检测筛查乳糜泻的临床价值 被引量:4

Clinical value of celiac disease screening by antibody combined with genetic testing

摘要 目的探讨乳糜泻抗体联合人类白细胞抗原(human leukocyte antigen,HLA)-DQ2、HLA-DQ8基因检测在筛查乳糜泻中的临床价值。方法对2017年1月至2018年5月于首都儿科研究所附属儿童医院消化内科就诊的腹痛、腹泻、呕吐、恶心持续2周以上患儿进行HLA-DQ2、HLA-DQ8基因检测以及血清组织型转谷氨酰胺酶抗体(tissue-type transfer effect of transglutaminase antibodies,tTGA)IgA、tTGA IgG、麦胶蛋白抗体(anti-gliadin antibody,AGA)IgA、AGA IgG抗体检查,收集患儿的临床资料。结果共纳入116例患儿,其中113例检测了HLA-DQ2、HLA-DQ8基因,有32.7%患儿基因检测阳性,包括3.5%患儿HLADQ8基因阳性,29.2%患儿HLA-DQ2基因阳性。116例患儿均检测了血清tTGA IgA、tTGA IgG、AGA IgA、AGA IgG抗体,其中tTGA IgA抗体阳性1例,无tTGA IgG抗体阳性病例,AGA IgA抗体阳性7例(其中有4例HLA-DQ2/8基因均阴性),AGA IgG抗体阳性2例(其中1例伴有HLA-DQ2基因阳性),1例tTGA IgA、AGA IgG、AGA IgA抗体均阳性,该例患儿的tTGA IgA抗体水平超过正常上限10倍且HLA-DQ2基因阳性。结论乳糜泻抗体联合HLA-DQ2、HLA-DQ8基因检测筛查乳糜泻具有重要的临床价值。AGA抗体检测存在假阳性可能,特异性不高,不建议用于乳糜泻的筛查。
