吉西他滨联合替吉奥对局部晚期鼻咽癌患者外周血γ-谷氨酰转移酶及免疫细胞亚群的影响

Effect of gemcitabine combined with tegiol on peripheral bloodγ-glutamyltransferase and immunocyte subsets in patients with locally advanced nasopharyngeal carcinoma

目的探讨吉西他滨联合替吉奥对局部晚期鼻咽癌患者外周血γ-谷氨酰转移酶(γ-GT)及免疫细胞亚群的影响。方法选择本院于2016年12月-2018年12月收治的局部晚期鼻咽癌患者62例,按照随机数字表法分为对照组31例与观察组31例。对照组给予吉西他滨联合顺铂治疗,观察组给予吉西他滨联合替吉奥胶囊治疗。2组均以21 d为1个周期,于化疗2个~4个周期。比较2组近期疗效,治疗前后KPS评分、外周血γ-GT和免疫细胞亚群变化及毒副反应发生情况。结果观察组总有效率(70.97%)高于对照组(41.94%)(P<0.05)。2组治疗后KPS评分较治疗前升高,观察组治疗后KPS评分高于对照组(t=7.442,P<0.05)。2组治疗后外周血γ-GT水平较治疗前降低,观察组治疗后外周血γ-GT水平低于对照组(t=15.455,P<0.05)。2组治疗后CD3^+、CD4^+和CD4^+/CD8^+较治疗前升高;观察组治疗后CD3^+、CD4^+和CD4^+/CD8^+高于对照组(t值分别为14、161、11.852、12.450,P<0.05)。观察组毒副反应发生率高于对照组,上述差异均有统计学意义(P<0.05)。结论吉西他滨联合替吉奥对局部晚期鼻咽癌患者近期疗效良好,可增强机体细胞免疫功能,降低外周血γ-GT水平,毒副反应少,值得临床借鉴。

Objective To investigate the effects of gemcitabine combined with tegiol on peripheral bloodγ-glutamyltransferase(γ-GT)and immunocyte subsets in patients with locally advanced nasopharyngeal carcinoma(NPC).Methods A total of62 patients with locally advanced nasopharyngeal carcinoma admitted to our hospital from December 2016 to December 2018 were divided into control group(31 cases)and observation group(31 cases)according to random number table method.The control group was treated with gemcitabine combined with cisplatin,while the observation group was treated with gemcitabine combined with tegio capsules.In both groups,21 days were used as a cycle,and 2-4 cycles of chemotherapy were used.The short-term efficacy,KPS score,changes of peripheral bloodγ-GT and immunocyte subsets,and occurrence of toxicity and side effects were compared between the two groups before and after treatment.Results The total effective rate of the observation group(70.97%)was higher than that of the control group(41.94%)(P<0.05).After treatment,the KPS score of the two groups was higher than that before treatment,and the KPS score of the observation group was higher than that of the control group(t=7.442,P<0.05).After treatment,the level of peripheral bloodγ-GT in the two groups was lower than that before treatment,and the level of peripheral bloodγ-GT in the observation group was lower than that in the control group(t=15.455,P<0.05).After treatment,CD3^+,CD4^+,and CD4^+/CD8^+in the two groups were higher than those before treatment,and CD3^+,CD4^+,and CD4^+/CD8^+in the observation group were higher than those in the control group(t=14,161,t=11.852,t=12.450,P<0.05).The incidence of toxic side effects in the observation group was higher than that in the control group,with the differences statistically significant(P<0.05).Conclusion Gemcitabine combined with Tegio has a good short-term effect on patients with locally advanced nasopharyngeal carcinoma.It can enhance the cellular immune function of the body,reduce the level ofγ-GT in peripheral blood,and have less toxic and side effects.It is worthy of clinical reference.
