桃红四物汤联合切开复位内固定术治疗早期跟骨关节内骨折

Clinical study of Taohong Siwu Decoction combined with open reduction and internal fixation for early intra-articular calcaneal fractures

目的探讨桃红四物汤联合切开复位内固定术治疗早期跟骨关节内骨折的临床效果。方法选择82例跟骨关节内骨折患者作为研究对象。随机将其分为对照组和研究组,各41例。2组均予切开复位内固定术治疗,其中对照组术后应用抗生素防感染,并进行烤灯理疗恢复;研究组在对照组的基础上给予桃红四物汤辅助治疗,连续治疗1个月。对比分析2组治疗前后足部功能评分、术后骨骼形态及临床疗效。结果治疗后,2组足部功能评分均升高,且研究组高于对照组(P<0.05);研究组术后根骨结节关节角(Bohler角)、根骨交叉角(Gissane角)均高于对照组(P<0.05);研究组疗效优良率(92.68%,38/41)显著高于对照组(63.41%,26/41)(P<0.05)。结论切开复位内固定术联合术后桃红四物汤加味治疗早期跟骨关节内骨折患者,临床疗效确切,能够减轻患者疼痛,促进患者骨骼形态的恢复及增强足部功能。

Objective To investigate the clinical effect of Taohong Siwu Decoction combined with open reduction and internal fixation on early intra-articular calcaneal fractures.Methods A total of 82 patients with intra-articular calcaneal fractures were selected and randomly divided into control group (41 cases) and research group (41 cases).Both groups were treated with open reduction and internal fixation.The control group received antibiotics and anti-infection after operation,and the ray physiotherapy was resumed.The patients in the research group were given the adjuvant treatment of Taohong Siwu Decoction on the basis of the control group.All of them were treated for one month continuously.The scores of the foot function,postoperative bone morphology and clinical efficacy between the two groups before and after treatment were observed and compared.Results After treatment,the scores of foot function increased in both groups,and the research group was higher than that in the control group (P<0.05).The postoperative Bohler angle and Gissane angle were higher in the research group than them in the control group,and the difference was obviously significant (P<0.05).The excellent and good rate in the research group was 92.68% (38/41),which was obviously higher than 63.41% (26/41) in the control group (P<0.05).Conclusion The clinical efficacy of open reduction and internal fixation combined with postoperative Taohong Siwu Decoction in patients with early intra-articular calcaneal fractures is definite,which can reduce the pain of patients,promote the recovery of bone morphology and enhance adequate function.
