腹腔镜下阑尾切除术与开腹阑尾切除术在穿孔性阑尾炎中的疗效及安全性分析

Analysis of the efficacy and safety of laparoscopic appendectomy and open appendectomy in perforated appendicitis

目的分析腹腔镜与开腹手术治疗穿孔性阑尾炎的临床效果。方法选取2015年1月至2016年12月就诊于本院的穿孔性阑尾炎患者70例,按照随机数字表法分为腹腔镜组与开腹组,各35例。腹腔镜组行腹腔镜手术切除,开腹组行开腹手术,比较两组手术效果。结果腹腔镜组患者切口长度及手术时间明显短于开腹组,术中出血量低于开腹组,差异有统计学意义(P<0.05);腹腔镜组患者术后住院时间、胃肠道功能恢复时间与排气时间均短于开腹组,差异有统计学意义(P<0.05);腹腔镜组患者术后并发症发生率低于开腹组,差异有统计学意义(P<0.05)。结论腹腔镜手术在穿孔性阑尾炎患者中的应用价值较高,可有效缩短患者术后恢复进程,促进患者早日康复,值得临床推广。

Objective To analyze the clinical effect of laparoscopy and laparotomy in the treatment of perforated appendicitis. Methods 70 patients with perforated appendicitis admitted to our hospital from January 2015 to December 2016 were divided into the laparoscopic group and the laparotomy group according to the random number table method, with 35 cases in each group. The laparoscopic group underwent laparoscopic surgery, and the laparotomy group underwent open surgery. The surgical results of the two groups were compared. Results The incision length and operation time in the laparoscopic group were significantly shorter than those in the laparotomy group, and the intraoperative blood loss was lower than that in the laparotomy group, with statistically significant difference(P<0.05). The postoperative hospital stay time, gastrointestinal function recovery time and exhaust time of patients in the laparoscopic group were shorter than those in the laparotomy group, with statistically significant difference(P<0.05). The incidence of postoperative complications in the laparoscopic group was lower than that in the laparotomy group, with statistically significant difference(P<0.05). Conclusion Laparoscopic surgery is of high application value in patients with perforated appendicitis, which can effectively shorten the postoperative recovery process and promote the early recovery of patients, and is worthy of clinical promotion.
