温针灸对膝关节骨性关节炎软骨下骨中OPG、RANKL蛋白表达的影响 被引量:14

Study on the Effect of Warm Acupuncture and Moxibustion on the Expression of OPG and RANKL Proteins in Subchondral Bone of Knee Osteoarthritis

摘要 目的观察温针灸对膝骨关节炎(KOA)模型兔软骨下骨中OPG、RANKL蛋白的影响,探讨温针灸治疗膝骨关节炎的效应机制。方法选取6月龄新西兰雄兔40只,经随机数字表法分为温针灸组、西药组、模型组、空白对照组,每组10只。实验干预后采用HE染色观察软骨及软骨下骨形态结构,免疫组化法检测各组软骨下骨组织中OPG、RANKL蛋白的表达。结果 (1)HE染色:空白组软骨层结构清晰完整;模型组软骨及软骨下骨受损严重,软骨下骨中软骨细胞排列紊乱,大小、密度不均;西药组与温针灸组较模型组软骨面光滑、完整,软骨下骨细胞分布较均匀。(2)免疫组化:OPG于空白组与模型组中表达差异无统计学意义(P>0.05),相对于空白组与模型组,温针灸组与西药组中OPG的表达均增高(P<0.01);RANKL于模型组的表达高于空白组、温针灸组和西药组(P均<0.05);温针灸组和西药组OPG/RANKL比例高于空白组与模型组(P均<0.01)。结论温针灸可促进软骨下骨中OPG蛋白表达,抑制RANKL蛋白表达,从而减缓软骨下骨骨重塑,达到治疗KOA的目的。 Objective To observe the effect of warm acupuncture on OPG and RANKL proteins in subchondral bone of rabbits KOA model,and explore the mechanism of warm acupuncture and moxibustion in the treatment of knee osteoarthritis.Methods A total of 40 New Zealand male rabbits aged 6 months were randomly divided into four groups:warm acupuncture group,western medicine group,model group and blank control group,10 in each group.After the experiment,HE staining was used to observe the morphology and structure of cartilage and subchondral bone,and immunohistochemistry was used to detect the expression of OPG and RANKL protein in subchondral bone.Results①The HE staining results showed that the structure of cartilage layer in blank group was clear and complete.In the model group,cartilage and subchondral bone were severely damaged,the arrangement of chondrocytes in the subchondral was disordered,and the size and density of chondrocytes were uneven.Compared with the model group,the cartilage surface of western medicine group and warm acupuncture group was smooth and complete,the distribution of cells in the subchondral bone layer was uniform.②The Immunohistochemistry results showed that there was no significant difference in OPG expression between the blank group and the model group(P>0.05).Compared with the blank group and model group,the expression of OPG in warm acupuncture group and western medicine group was significantly higher(P<0.01).The expression of RANKL in model group was higher than that in blank group(P<0.05),warm acupuncture group(P<0.05)and western medicine group(P<0.05).The proportion of OPG/RANKL in the warm acupuncture group was higher than that in the blank group(P<0.05)and the model group(P<0.01).Conclusion Warm acupuncture can promote the expression of OPG protein in subchondral bone and inhibit the expression of RANKL protein,so as to slow down the remodeling of subchondral bone and achieve the purpose of treating KOA.
